摘要
目的:观察中药百合知母汤加味治疗围绝经期抑郁(PPD)的临床疗效。方法:选择我院2018年1月至2019年6月收治的PDD患者90例作为研究对象,按照随机数字表法分为2组,每组45例,对照组给予氟哌噻吨美利曲辛片口服,每次1片,每日2次;治疗组在对照组用药基础上应用百合知母汤加味,每日1剂,早晚分2次温服,两组均连续治疗8周。比较两组治疗前后汉密尔顿抑郁量表(HAMD)评分、更年期综合征(Kupperman)评分,并比较两组血雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)、促肾上腺皮质激素(ACTH)、肾上腺皮质酮(CORT)、5-羟色胺(5-HT)、脑源性神经营养因子(BDNF)水平。并比较两组临床疗效。结果:治疗后治疗组HAMD、Kupperman评分均低于对照组(P<0.01);E2、5-HT、BDNF水平高于对照组(P<0.01),FSH、LH、ACTH、CORT水平低于对照组(P<0.01),临床疗效优于对照组(P<0.05),差异均有统计学意义。结论:百合知母汤加味可调节性激素分泌、增加5-HT、BDNF表达,降低ACTH、CORT的表达,改善患者的抑郁症状,治疗PDD效果显著。
Objective:To investigate the clinical effect of Baihe Zhimu Decoction on treating perimenopausal depression(PPD).Methods:Ninety patients with PDD admitted to our hospital from January 2018 to June 2019 were selected as the research objects.They were divided into two groups according to the random number table method,with 45 cases in each group.The controlled group were given Halopethiazide-Ton Melitracen Tablets orally,one tablet each time,twice a day.The treatment group were given Baihe Zhimu Decoction on the basis of the controlled group,one dose a day,twice a day in the morning and evening.The two groups were treated for 8 weeks.Hamilton Depression Scale(HAMD) score and Kupperman score before and after treatment were compared between the two groups,and blood estradiol(E2),follicle stimulating hormone(FSH),luteinizing hormone(LH),adrenocorticotropic hormone(ACTH),adrenocorticosterone(CORT),5-hydroxytryptamine(5-HT),brain source were compared between the two groups.Sexual neurotrophic factor(BDNF) level.The clinical effects of the two groups were compared.Results:After treatment,HAMD and Kupperman scores of the treatment group were lower than those of the controlled group(P<0.01),E2,5-HT and BDNF levels were higher than those of the controlled group(P<0.01),FSH,LH,ACTH and CORT levels were lower than those of the controlled group(P<0.01),and the clinical efficacy was better than that of the controlled group(P<0.05),the difference was statistically significant.Conclusion Baihe Zhimu Decoction can regulate the secretion of sex hormones,increase the expression of 5-HT and BDNF,reduce the expression of ACTH and CORT,improve the depressive symptoms of patients,and it has a significant effect in the treatment of PDD.
