摘要
目的:观察益平调脉方治疗阻塞性睡眠呼吸暂停低通气综合征合并高血压的临床价值。方法:选取我院2018年2月~2019年5月64例确诊为阻塞性睡眠呼吸暂停低通气综合征合并高血压的患者,按随机数字表法分为观察组(n=32)和对照组(n=32),对照组使用常规治疗,观察组在对照组基础上给予益平调脉方治疗。比较两组临床疗效、治疗前后症状积分、动态血压变化情况、血管活性物质水平、通气情况。结果:观察组临床治疗有效率90.63%高于对照组68.75%(P<0.05);观察组治疗后嗜睡、打鼾、眩晕、心悸、口中乏味、食少、头重如裹等症状积分均低于对照组(P<0.05);观察组治疗后睡眠呼吸暂停低通气指数均低于对照组,最长呼吸暂停时间短于对照组(P<0.05);观察组治疗后白天、夜晚、24 h平均舒张压、收缩压均低于对照组(P<0.05);观察组治疗后醛固酮、血管紧张素Ⅱ、肾素水平低于对照组(P<0.05)。结论:益平调脉方治疗阻塞性睡眠呼吸暂停低通气综合征合并高血压的临床疗效显著,可以缓解临床症状,调节患者血压,改善呼吸情况。
Objective:To explore the clinical value of using Yiping Tiaomai Prescription in the treatment of obstructive sleep apnea-hypopnea syndrome(OSAHS)and hypertension.Methods:64patients who were confirmed with OSAHS and hypertension in the hospital from February 2018to May 2019were enrolled.They were divided into observation group(n=32)and controlled group(n=32)by random number table method.The controlled group was given routine therapy,while the observation group was given Yiping Tiaomai Prescription on the basis of the controlled group.The clinical curative effect,symptom scores,changes of dynamic blood pressure,levels of vasoactive substances and ventilation before and after treatment were compared between the two groups.Results:The response rate of clinical treatment in the observation group was higher than that in the controlled group(90.63%vs 68.75%)(P<0.05).After treatment,scores of symptom such as sleepiness,snoring,dizziness,palpitation,vapidity in mouth,deficiency of food and head heaviness in the observation group were lower than those in the controlled group(P<0.05).After treatment,sleep apnea-hypopnea indexes in the observation group were lower than those in the controlled group,and the longest apnea time was shorter than that in the controlled group(P<0.05).After treatment,daytime,night and 24h average diastolic blood pressure and systolic blood pressure in the observation group were lower than those in the controlled group(P<0.05).After treatment,levels of aldosterone,angiotensin II and renin in the observation group were lower than those in the controlled group(P<0.05).Conclusion:The clinical curative effect of Yiping Tiaomai Prescription is significant on OS-AHS and hypertension,which can relieve clinical symptoms,regulate blood pressure and improve breathing of patients.
