摘要
目的:考察儿童及青少年版三水平五维健康量表[EQ-5D-Y-3L(3LY)]和五水平五维健康量表[EQ-5D-Y-5L(5LY)]用于评价血液肿瘤儿童的健康相关生命质量时的可行性和效度。方法:通过对72例血液肿瘤儿童进行面对面的EQ-5D-Y问卷调查,并用量表各维度与美国东部肿瘤协作组织(ECOG)评分的相关性检验量表的效度。结果:72例患儿平均年龄为(10.4±2.2)岁,其中男孩占66.7%,全部患儿的平均视觉模拟量表(EQ-VAS)得分为86.0±15.3;3LY到5LY的再分布不一致率介于0.0%~11.1%;3LY和5LY两个量表的"行动能力""自我照顾""日常活动""疼痛或不舒服"4个维度在病情良好组和病情不佳组间的差异均有统计意义(P<0.05),病情不佳组报告了更多的问题,EQ-VAS得分也更低;EQ-VAS与ECOG评分中等相关(|rs|=0.44),3LY和5LY的"担心、伤心或不高兴"与ECOG低相关(|rs|<0.29),5LY的"行动能力"与ECOG强相关(|rs|=0.58),3LY和5LY的其他维度与ECOG中等相关(0.33≤|rs|≤0.49)。结论:EQ-5D-Y-3L和EQ-5D-Y-5L都能很好地区分各种健康状态,用于评价血液肿瘤儿童生命质量具有一定的可行性和效度。
Objective: To investigate the feasibility and validity of EQ-5 D-Y-3 L( 3 LY) and EQ-5 D-Y-5 L( 5 LY) in evaluating health-related quality of life in children with hematological malignancies. Methods: A face-to-face EQ-5 D-Y questionnaire interview was conducted among 72 children with hematological malignancies. The validity of the scale was tested by the correlation between the dimensions of the scale and ECOG. Results: The average age of 72 children was( 10.4±2.2) years,of which 66.7% were boys,and the average EQ-VAS of children was 86.0 ± 15.3. The rate of inconsistency of redistribution of 3 LY to 5 LY was between 0.0% and11.1%. There was significant statistical difference between the good condition group and poor condition group in the"mobility","looking after myself","usual activities"and"pain/discomfort"four dimensions of 3 LY and 5 LY( P< 0.05). The poor condition group reported more problems and the EQ-VAS was lower. EQ-VAS was moderately correlated with ECOG( | rs| = 0.44). The dimension"feeling worried/sad/unhappy"of 3 LY and 5 LY had low correlation with ECOG( |rs| <0.29). The dimension"mobility"of 5 LY had strong correlation with ECOG( | rs| = 0.58). Other dimensions of 3 LY and 5 LY were moderately correlated with ECOG( 0.33≤ | rs| ≤0.49). Conclusion: EQ-5 D-Y-3 L and EQ-5 D-Y-5 L can distinguish all kinds of health states very well. It is feasible and valid to evaluate the quality of life of children with hematological malignancies.
