摘要
目的 评估表观扩散系数值(ADC值)对胆管癌转移淋巴结的诊断价值.方法 回顾性分析32例胆管癌患者的磁共振成像(MRI)资料,测量淋巴结的ADC值和短轴直径,使用t检验进行比较,建立受试者工作特征曲线(ROC)确定阈值.结果 共纳入116个淋巴结,其中74个良性淋巴结和42个转移淋巴结.良性淋巴结的平均短轴直径(7.5±1.9)mm明显小于转移性淋巴结(10.3±2.5)mm,最佳阈值为0.86,诊断准确率为72%.转移性淋巴结的ADC值(1.42×10-3 mm2/s)与良性淋巴结(1.52×10-3 mm2/s)差异无统计学意义.结论 ADC值对胆管癌患者良性和转移性淋巴结的鉴别诊断价值有限.
Objective To investigate diagnostic value of ADC value in metastatic lymph nodes of cholangiocarcinoma.Methods Retrospective analysis of MRI data of 32 patients with cholangiocarcinoma was performed.The ADC values and short axis diameters of the lymph nodes were measured and compared using a t-test.ROC curves were used to determine the threshold.Results A total of 116 lymph nodes were included-74 benign lymph nodes and 42 metastatic lymph nodes.The mean short-axis diameter of the benign lymph nodes(7.5±1.9)mm was significantly smaller than that of the metastatic lymph nodes(10.3±2.5)mm,and the optimal threshold was 0.86.The diagnostic accuracy was 72%.There was no significant difference in ADC values between metastatic lymph nodes(1.42×10-3 mm2/s)and benign lymph nodes(1.52×10-3 mm2/s).Conclusion ADC value is of limited value in differential diagnosis of benign and metastatic lymph nodes in patients with cholangiocarcinoma.
