摘要
目的总结6例复杂胫骨平台骨折合并腘血管损伤患者的围手术期观察与护理方法。方法对2011年7月至2019年1月本院收治的6例复杂胫骨平台骨折合并腘血管损伤患者实施病情评估、体位护理、康复锻炼指导等护理措施,并对其中合并严重并发症伴有发生血管损伤和肌肉坏死、肾功能损害、脂肪栓塞综合征的手术患者进行对应的围手术期观察与护理。结果5例患者治愈出院,1例患者术后合并脂肪栓塞综合征死亡。结论病情评估对可能发生的并发症做到早期的处理和预防;体位护理在后正中入路治疗胫骨平台骨折中可提高手术效果;康复锻炼指导促进肢体功能的恢复;重点关注合并发生血管损伤和肌肉坏死、肾功能损害、脂肪栓塞综合症,均是临床重要的护理经验。
Objective To summarize the effect of nursing care for patients with complicated tibial plateau fracture combined with popliteal vascular injury.Methods Six patients with complicated tibial plateau fracture and vascular injury in our hospital from July 2011 to January 2019 were given the nursing measures of condition evaluation,position nursing and rehabilitation exercise guidance.The perioperative observation and nursing were carried out for the patients with severe complications,vascular injury,muscle necrosis,renal function damage and fat embolism syndrome.Result Five of them were cured and discharged but 1 case died of fat embolism syndrome after operation.Conclusions Early treatment and prevention of possible complications should be carried out by way of disease assessment.Position nursing improves the operative effect in the treatment of tibial plateau fracture through posterior median approach.Rehabilitation exercise guidance promotes the recovery of body function.The nursing measures for complications like vascular injury,muscle necrosis,renal function damage and fat embolism syndrome are important and instructive.
