慢性阻塞性肺疾病急性加重期患者血清中N末端B型利钠肽原水平与肺功能、生活质量的相关性分析 被引量:9

Correlation between serum levels of NT-proBNP and pulmonary function,quality of life in patients with acute exacerbation of COPD

摘要 目的探讨血浆NT-proBNP水平与慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能及生活质量的关系。方法选取本院呼吸内科收治的AECOPD患者78例,按照血浆NT-proBNP水平分为升高组31例和正常组47例。比较2组患者的用力呼气肺活量(FVC)、用力呼气一秒量(FEV1)、用力呼气一秒率(FEV1/FVC)、最大呼气中段流速(MMEF)及呼气峰值流速(PEF);采用SF-36量表评价生活质量;对NT-proBNP水平与肺功能指标关系进行相关性分析。结果升高组患者FVC、FEV1和FEV1/FVC均较正常组降低,MMEF和PEF较正常组减慢,差异有统计学意义(P<0.05)。升高组患者SF-36量表的8个维度评分均较正常组降低,差异有统计学意义(P<0.05)。血浆NT-proBNP水平与患者LVEF、FVC、FEV1、FEV1/FVC、MMEF、PEF呈负相关(P<0.05)。结论NT-proBNP水平同AECOPD患者肺功能指标和生活质量关系密切,可为病情监测提供依据。 Objective To investigate the relationship between plasma NT-proBNP level and lung function,quality of life in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Seventy-eight patients with AECOPD admitted to our department of respiratory medicine were enrolled.The plasma NT-proBNP levels were divided into elevated group(n=31)and normal group(n=47).Comparison of forced expiratory vital capacity(FVC),forced expiratory volume(FEV1),forced expiratory second rate(FEV1/FVC),maximum expiratory mid-flow velocity(MMEF),and peak expiratory flow rate(PEF)were compared between the two groups.The quality of life was evaluated by SF-36 scale;the correlation between NT-proBNP level and lung function index was analyzed.Results In the elevated group,FVC,FEV1 and FEV1/FVC were lower than those in the normal group,and MMEF and PEF were slower than the normal group,with the differences statistically significant(P<0.05).The eight-dimensional scores of the SF-36 scale in the elevated group were lower than those in the normal group,with the differences statistically significant(P<0.05).Plasma NT-proBNP levels were negatively correlated with LVEF,FVC,FEV1,FEV1/FVC,MMEF,and PEF(P<0.05).Conclusion The level of NT-proBNP is closely related to the lung function index and quality of life of AECOPD patients,which can provide basis for disease monitoring.
