摘要
目的:比较改良小夹板与传统小夹板固定桡骨远端伸直型骨折复位后的临床疗效。方法:将选取的113例桡骨远端伸直型骨折患者,分成观察组55例和对照组58例,两组在手法整复后分别采用改良小夹板固定、传统小夹板固定,比较两组患者在治疗4周内夹板调整次数、疼痛缓解时间、手腕消退时间、骨折临床愈合时间及夹板固定后并发症情况,两组在治疗3个月后掌倾角和尺偏角的丢失值,Green和O’Brien腕关节功能评分,治疗后优良率。结果:观察组与对照组在4周内夹板调整次数分别为(2.58±0.78)次、(4.03±0.81)次。观察组与对照组疼痛缓解时间分别为(3.13±1.01)d、(5.23±1.25)d,手腕肿胀消退时间分别为(5.62±1.02)d、(7.24±2.12)d,骨折愈合时间分别为(36.17±3.12)d、(46.14±3.56)d,差异均有统计学意义(P<0.05)。观察组与对照组3月后的掌倾角丢失值分别为(1.2±1.46)°、(1.79±1.82)°,尺偏角丢失值(1.68±1.37)°、(1.52±2.16)°,两组比较差异无统计学意义(P>0.05),两组Green和O’Brien评分分别为(89.78±11.52)分、(80.39±13.28)分,两组比较差异有显著统计学意义(P<0.05)。观察组优良率90.9%,对照组优良率81.0%,观察组患者并发症发生率低于对照,差异有统计学意义(P<0.05)。结论:改良夹板固定桡骨远端伸直型骨折优于传统小夹板,具有良好实用效果,值得临床推广使用。
Objective:To compare the clinical effects of the modified small splint with traditional small splint on the distal radius extension fractures.Methods:113 patients with distal radius extension fractures were divided into observation group(55 cases)and comparative group(58 cases).The observation group was treated with modified small willow splint,and the comparative group was treated with traditional small splint.To observe the number of the two groups of splint adjusting within 4 weeks,the time of wrist swelling to subsides、pain relief、fracture healing and complications after splint fixation.The missing values of volar tilt angles and ulnar inclination angles,Green and O’Brien wrist function scores,the excellent and good rate after treatment were recorded and compared after 3 months.Results:The number of splint adjusting within 4 weeks between the two group respectively were(2.58+0.78)times、(4.03+0.81)times.Pain relief time were(3.13±1.01)d,(5.23±1.25)d,wrist swelling regression time were(5.62±1.02)d,(7.24±2.12)d,fracture healing time were(36.17±3.12)d、(46.14±3.56)d.The difference between the observation group and the control group was statistically significant(P<0.05).The missing values of volar tilt angles and ulnar inclination angles after 3 months in the observation group and the control group respectively were(1.2±1.46)°(1.79±1.82)°,missing values of ulnar deviation Angle(1.68±1.37)°and(1.52±2.16)°.There was no significant difference between the two groups.The excellent and good rate was 90.9%in the observation group and 81.0%in the control group,The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:The modified splint is better than the traditional splint in fixing the distal radius extension fracture,it is worth clinical promotion.
