摘要
目的:了解应用自体浓缩生长因子凝胶联合发泡表皮移植用于修复难愈性创面的可行性及其治疗效果。方法:选择16例难愈性创面的住院患者,共16处且持续6周以上未愈合的,进行浓缩生长因子凝胶膜联合自体发泡表皮移植修复,治疗方法为I期创面扩创,填塞凝浓缩生长因子凝胶,根据创面凝胶溶解情况,7~10天更换一次浓缩生长因子凝胶,3周后创面肉芽组织生长、新鲜。Ⅱ期手术,利用皮肤分离仪于腹壁发泡,取数个直径1cm2表皮并间隔覆盖于创面,浓缩生长因子凝胶膜覆盖。结果:16处难愈性创面患者中14处创面愈合良好,2处创面部分皮片成活欠佳、溶解,残余创面经换药后愈合。创面愈合后随访18个月,仅3例出现张力性水泡,部分色素沉着,均未见溃疡复发,无明显增生性瘢痕。腹壁发泡取皮区皮肤颜色正常,质软,未见瘢痕生长。结论:浓缩生长因子凝胶联合自体发泡表皮移植修复难愈性创面,因其取材方便,方法简单、供皮区损伤小、创面愈合质量高,宜在临床上推广,为难愈性创面的修复提供新的治疗方法。
Objective:To observe the feasibility of Repair non-healing woundwith Concentrated growth factor(CGF)andAutologous epidermis,and to evaluate its efficacy.Methods:Choose 16 inpatients with 16 refractory wounds lasting more than 6 weeks,Concentrated growth factor combined with autologous epidermal graft to repairwound,Wounds were debrided and repaired with Concentrated growth factor(CGF)in the first stage,According to the wound Concentrated growth factor dissolution,Change concentrated growth factor 7 to 10 days,after 3 weeks,the granulation tissue grows and fresh.Use Skin separatorinstrumentto foam in the abdominal wall,take a number of 1cm2 epidermis and Interval coverage of the wound,and covered with concentrated growth factor.Results:16 cases of chronic wounds,14 patients with excellent wound healing,two wound heal after dressing change.Patients were followed up for 18months,Wounds healed with no recurrence of wounds,satisfactory appearance,no obvious hypertrophic scars,normal skin color of the abdominal wall foaming area,no scar growth.Conclusions:Repair refractory woundwith Concentrated growth factor(CGF)and Autologous epidermisis simple;and it results in healing of wounds with high quality and little damage to the donor site.It provides a new choice for repairing non—healing wound.
