摘要
总结彭玉教授运用运脾益肾法辨治小儿疳证的经验。彭教授用察体观色诊察方法,归纳四大证候群(饮食不节、形体羸瘦、大便不调、精神异常)识别证候体征,分析小儿疳证生长缓慢的病因病机;用5类证候(脾胃虚弱证、胃强脾弱证、脾虚肝旺证、脾肾不足证、阴虚内热证)法辨证,通过论虚实,以判断脏腑的转归。凡夹实证者,宜先治实,后补虚;虚实夹杂者,宜分辨虚实孰轻孰重,决定补虚祛邪孰多孰少。自拟运脾益肾方补运兼施,寓补于运,运中有补,集运脾益肾、消滞化积、益气于一方,体现了运脾益肾、调脾肾、助生长之意,临床疗效显著。附典型案例1则,以资验证。
This paper summarizes professor PENG Yu’experience in using activating the spleen and tonifying the kidney method in differential treatment of infantile malnutrition.Professor PENG uses the method of diagnosis and examination by inspecting body and complexion,generalizes four symptom complex(improper diet,emaciation,fecal disorders and insanity) to identify symptoms and signs,and analyses the etiology and pathogenesis of growth retardation of infantile malnutrition.Using 5 categories of symptoms(weakness of the spleen and the stomach symptom,strong stomach and weak spleen symptom,weak spleen and effulgence liver symptom,insufficiency of the spleen and the kidney symptom and yin deficiency with internal heat symptom)methods to differentiate symptoms and signs,by discussing deficiency and excess to judge the prognosis of zang and fu organs.Those complicated by excess symptom, should treatment of excess come before strengthening asthenia;those deficiency mingling with excess,should distinguish deficiency from excess and the order of priority,and determine reinforcing deficiency and expelling pathogenic factors and the quantity of each.Self-made Yunpi Yishen Fang(运脾益肾方) uses reinforcing and transportation in combination, supplementation involving transportation,existence of reinforcing in transportation,integrating activating spleen and nourishing kidney,removing retention and resolving qi stagnation and replenishing qi into one,which embodies the meaning of activating spleen and nourishing kidney,harmonizing spleen and kidney,and strengthening the growth with significant clinical efficacy.One typical case is attached for verification.
