摘要
目的探讨中心静脉压(CVP)测量对急危重症患者的意义,并分析CVP测量准确性的影响因素。方法选取于2015年6月-2018年9月在焦作市第二人民医院收治的危急重症患者65例为研究对象,纳入观察组,同时选取65例健康志愿者纳入对照组,比较两组入院第1 d、3 d、6 dCVP偏离正常值水平,并分析其测量准确性的影响因素。结果入院后第1 d、3 d、6 d观察组CVP水平(3.76±1.42)cmH2O、(2.25±1.03)cmH2O、(1.86±0.05)cmH2O与对照组比较差异有统计学意义(P<0.05),且观察组CVP偏离值有减小趋势(P<0.05),至入院第10 d与对照组比较差异无统计学意义(P>0.05);CVP测量准确性受呼气末正压、体位、是否使用血管活性药物及输液速度等因素影响,不同液体种类、输液器类型下CVP测量准确性差异无统计学意义(P>0.05)。结论急危重症患者CVP测量的准确性受呼气末正压、体位、血管活性药物、输液速度等因素影响,临床在检测CVP时需重视。
Objective To investigate the significance of measurement ofcentral venous pressure(CVP) in patients with acute and severe disease, and to analyze the influencing factors for the accuracy of CVP measurement. Methods 65 cases of patients with acute and severe disease who were admitted to the hospital from June,2015 to September,2018 were selected as study subjects and were enrolled in the observation group. Another 65 healthy volunteers were enrolled in the control group. The CVP deviating from the normal level was compared between the two groups on the 1 st, 3 rd and 6 th day after admission. The factors influencing the accuracy of measurement were analyzed. Results On the 1 st, 3 rd and 6 th day after admission, CVP levels in observation group were(3.76±1.42) cmH2 O,(2.25±1.03) cmH2 O and(1.86± 0.05) cmH2 O respectively. Compared with those in the control group, there were significant differences(P<0.05) and the CVP deviation value in observation group showed a decreasing trend(P<0.05). On the 10 th day after admission, there was no significant difference between the two groups(P>0.05). The measurement accuracy of CVP was affected by positive end-expiratory pressure, body position, whether use vasoactive drugs and infusion speed, etc. There were no significant differences in measurement accuracy of CVP between different types of liquid and different types of infusion apparatus(P>0.05). Conclusion The measurement accuracy of CVP in patients with acute and severe disease is affected by positive end-expiratory pressure, body position, vasoactive drugs and infusion speed, etc., which should be paid attention to in the clinical detection of CVP.
