实时三维超声心动图和二维斑点追踪成像评价重度子痫前期左心房形态及功能 被引量:7

Real-Time Three-Dimensional Echocardiography and Two-Dimensional Speckle Tracking Imaging in Evaluating Left Atrial Morphology and Function in Severe Pre-eclampsia

摘要 目的应用实时三维超声心动图(RT-3DE)和二维斑点追踪成像(2D-STI)评估重度子痫前期(SPE)患者的左心房形态及功能。资料与方法选取45例SPE患者作为SPE组,43例健康孕妇作为对照组。采用RT-3DE获取左心房容积参数最大、最小及收缩前容积(LAVmax、LAVmin及LAVpre);采用2D-STI获取左心房各时相应变:储器期(LASr)、管道期(LAScd)及泵期应变(LASct)。比较两组参数的差异,分析相关参数与左心房各时相应变之间的相关性。结果与对照组比较,SPE组LAVmax、LAVmin及LAVpre较高,LASr及LAScd较低,差异有统计学意义(P<0.05)。多因素回归分析显示,收缩压、左心室整体长轴应变为LASr(β=-0.313、0.359,P<0.01)和LAScd(β=-0.416、0.349,P<0.01)的独立预测因素。结论RT-3DE和2D-STI可定量评估SPE的左心房形态及功能。 Purpose To evaluate the left atrial morphology and function of patients with severe pre-eclampsia(SPE)using real-time three-dimensional echocardiography(RT-3DE)and two-dimensional speckle tracking imaging(2D-STI).Materials and Methods Forty-five SPE patients were selected as the SPE group,and 43 healthy pregnant women were selected as the control group.RT-3DE was used to obtain LA maximum volume,minimal volume and volume prior to atrial contraction(LAVmax,LAVmin and LAVpre);the 2D-STI was used to obtain the phasic LA strain including LA reservoir strain,conduit strain and contraction strain(LASr,LAScd and LASct).The differences between the two groups were compared,and the correlation between relevant indices and LA phasic strain were analyzed.Results Compared with the control group,LAVmax,LAVmin and LAVpre were increased in SPE group(all P<0.05).LASr and LAScd were lower than that in the control group(all P<0.05).Multiple regression analysis showed systolic blood pressure and left ventricular global longitudinal strain were independently associated with LASr(β=-0.313,0.359,P<0.01)and LAScd(β=-0.416,0.349,P<0.01).Conclusion RT-3DE and 2D-STI can quantitatively evaluate the left atrial morphology and function of SPE.
