摘要
目的观察丹参注射液联合丙氨酰谷氨酰胺辅助治疗中重度急性胰腺炎的临床效果。方法将61例中重度急性胰腺炎患者随机分为对照组29例和研究组32例。两组均按规范给予常规治疗,在此基础上,研究组加用丹参注射液联合丙氨酰谷氨酰胺注射液治疗,对照组给予等量5%葡萄糖注射液及氨基酸注射液治疗,疗程均为7 d。记录两组患者腹胀缓解时间、肠鸣音恢复时间、恢复排便时间及住院天数,于入院24 h和治疗7 d时测定两组患者白细胞计数、C反应蛋白(CRP)、降钙素原、内毒素、D-二聚体、内皮素、白蛋白水平。结果研究组腹胀缓解时间、肠鸣音恢复时间、恢复排便时间、住院天数均短于对照组(均P<0.05)。治疗7 d后,研究组患者白细胞计数、CRP、降钙素原、内毒素、D-二聚体及内皮素水平均低于对照组,白蛋白水平高于对照组(均P<0.05)。结论丹参注射液联合丙氨酰谷氨酰胺辅助治疗中重度急性胰腺炎患者,可改善微循环障碍,维持肠道黏膜屏障,减轻内毒素血症,降低机体炎症反应水平,提高临床疗效。
Objective To observe the clinical effect of Danshen Injection combined with alanine glutamine in the adjuvant treatment of moderate to severe acute pancreatitis.Methods Sixty-one patients with moderate to severe acute pancreatitis were randomly divided into control group(29 cases)and study group(32 cases).Conventional therapy was performed in both groups in accordance with the specifications,based on which the study group was given Danshen Injection combined with Alanine Glutamine Injection additionally,whereas the control group was given the equivalent volume of 5%Glucose Injection plus Amino Acid Injection,and the course of treatment was seven days in both groups.Duration to abdominal distension remission,duration to normal bowel sound,duration to normal defecation,and hospital stay were recorded in both groups,the levels of WBC counts,C-reactive protein(CPR),procalcitonin,endotoxin,D-dimer,endothelia and albumin were detected in the two groups 24 hours after admission and seven days after treatment.Results The duration to abdominal distension remission,duration to normal bowel sound,duration to normal defecation,and hospital stay in the study group were shorter than those in the control group(all P<0.05).Seven days after treatment,the study group exhibited lower levels of WBC counts,CPR,procalcitonin,endotoxin,D-dimer and endothelia as well as a higher albumin level as compared with the control group(all P<0.05).Conclusion Danshen Injection combined with alanine glutamine can improve microcirculation disorder,maintain Intestinal mucosal barrier,ameliorate endotoxemia,reduce the level of inflammation response in the body,and improve clinical efficacy in the adjuvant treatment of patients with moderate to severe acute pancreatitis.
