摘要
目的探究不同病程2型糖尿病(T2DM)患者血清高敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、白细胞介素-18(IL-18)等炎症因子水平及其与临床参数的相关性。方法选择2018年3月至2018年12月我院收治的132例T2DM患者作为研究对象,按照糖尿病病程分为A组(病程<5年,35例)、B组(病程5~15年,50例)、C组(病程>15年,47例);另择同期的50例健康成年人作为对照组。收集研究对象临床资料,测定血清hs-CRP、IL-6、IL-18指标水平并分析其中的相关性。结果A、B、C组的BMI、并发症种数、SBP、DBP、FPG、2 h PG、HbA1c、HOMA-IR、TG、LDL-C、hs-CRP、IL-6、IL-18显著高于对照组,HDL-C显著低于对照组,差异具有统计学意义(P<0.05);B、C组的年龄、病程、并发症种数、FPG、2 h PG、HOMA-IR、hs-CRP、IL-6、IL-18显著高于A组,差异具有统计学意义(P<0.05);C组的年龄、病程、并发症种数、2 h PG、HbA1c、HOMA-IR、TC、LDL-C、hs-CRP、IL-6、IL-18显著高于B组,BMI显著低于B组,差异具有统计学意义(P<0.05)。Pearson相关性分析示,血清hs-CRP与病程、并发症种数、HOMA-IR呈正相关(r=0.352、0.307、0.401,P<0.05),血清IL-6水平与病程、HOMA-IR呈正相关(r=0.345、0.318,P<0.05),血清IL-18水平与HOMA-IR呈正相关(r=0.455,P<0.05)。结论血清hs-CRP、IL-6、IL-18水平随T2DM病程延长而升高,其水平与并发症种数、胰岛素抵抗及高脂血症等相关。
Objective To investigate the levels of serum high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),interleukin-18(IL-18)and their correlation with clinical parameters in patients with type 2 diabetes(T2DM)with different disease course.Methods A total of 132 T2DM patients admitted in our hospital from March 2018 to December 2018 were selected as the research subjects.According to the course of diabetes,the patients were divided into group A(disease course<5 years,35 cases)and group B(disease course 5 to 15 years,50 cases),group C(disease course>15 years,47 cases);another 50 healthy adults in the same period were selected as the control group.The clinical data of study objects were collected,and the levels of serum hs-CRP,IL-6 and IL-18 were measured and their correlation was analyzed.Results The BMI,number of complications,SBP,DBP,FPG,2 h PG,HbA1c,HOMA-IR,TG,LDL-C,hs-CRP,IL-6 and IL-18 in the group A,B,C were significantly higher than those in the control group,HDL-C was significantly lower than that in the control group,the differences were statistically significant(P<0.05);the age,course of disease,number of complications,FPG,2 h PG,HOMA-IR,hs-CRP,IL-6,IL-18 in the group B and C were significantly higher than those in the group A,the differences were statistically significant(P<0.05);the age,course of disease,number of complications,2 h PG,HbA1c,HOMA-IR,TC,LDL-C,hs-CRP,IL-6,and IL-18 in the group C were significantly higher than those in the group B,and BMI was significantly lower than that in the group B,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum hs-CRP was positively correlated with disease course,number of complications,and HOMA-IR(r=0.352,0.307,0.401,P<0.05),serum IL-6 level was positively correlated with disease course and HOMA-IR(r=0.345,0.318,P<0.05),and serum IL-18 level was positively correlated with HOMA-IR(r=0.455,P<0.05).Conclusion Serum hs-CRP,IL-6,and IL-18 levels increased with the duration of T2DM,and their levels were related to the number of complications,insulin resistance,and hyperlipidemia.
