摘要
目的观察痛风清热方治疗急性痛风性关节炎的临床疗效并探讨其对血清炎性因子的影响。方法按随机数字表法将56例急性痛风性关节炎患者分为治疗组和对照组,每组各28例。对照组予塞来昔布胶囊(0.2 g,每日2次)内服,治疗组在其基础上加服痛风清热方,疗程均为1周。医治前后分别测定患者静脉血清炎症因子[血尿酸(BUA)、红细胞沉降率(ESR)、高敏C反应蛋白(hs-CRP)、白细胞介素(IL)-1β、肿瘤坏死因子(TNF)-α)]的水平,记录并比较临床症状及生化指标的变化。结果治疗组总有效率高于对照组(P<0.05),治疗组静脉血BUA、ESR、CRP指标较对照组明显降低(P<0.01);治疗组IL-1β、TNF-α水平降低的幅度明显大于对照组(P<0.01)。结论痛风清热方治疗急性痛风性关节炎疗效确切、不良反应低,作用机制可能与降低IL-1β、TNF-α等炎症趋化因子有关,该方能显著改善临床症状,值得应用推广。
Objective To observe the clinical effect of Tongfeng Qingre formuda on acute gouty arthritis and to explore its influence on serum inflammatory factors.Methods Fifty-six patients with acute gout arthritis were divided into treatment group and control group according to the random number table method,28 patients in each group.The control group was given celecoxib capsule(0.2 g,bid),and the treatment group was given Tongfeng Qingre formula on the basis of it.The course of treatment was one week.The levels of serum inflammatory factors(BUA,ESR,hs-CRP,IL-1,TNF-α)were measured before and after treatment.The changes of clinical symptoms and bio-chemical indexes were recorded and compared.Result The total effective rate of the treatment group was higher than that of the control group(P<0.05).The indexes of BUA,ESR and CRP in the treatment group were significantly lower than those in the control group(P<0.01).The level of IL-1 and TNF-αin the treatment group was significantly lower than that in the control group(P<0.01).Conclusion The clinical effect of Tongfeng Qingre decoction on acute gouty arthritis is definite with low adverse reactions.The mechanism of action may be related to the reduction of in-flammatory chemokines such as IL-1 and TNF-α.This decoction can significantly improve the clinical symptoms and is worth popularizing.
