摘要
目的:观察和比较铜砭刮痧联合桂香温经止痛胶囊与单纯口服桂香温经止痛胶囊治疗寒凝血瘀型子宫内膜异位症痛经的临床疗效差异。方法:56例符合纳入标准的寒凝血瘀型子宫内膜异位症痛经患者,随机分为治疗组和对照组各28例,对照组予单纯桂香温经止痛胶囊治疗,主要成分为山药、白芍、小茴香、官桂、细辛、延胡索、五灵脂、川牛膝、当归、水蛭、山茱萸等,每日3次,每次4~6粒口服。治疗组在对照组基础上加铜砭刮痧法治疗,主要以督脉、八髎穴、臀部、带脉、小腹、腹股沟为重点刮痧治疗部位,每周治疗1次,1个月经周期为1个疗程,经期正常口服药物及刮痧治疗,两组均治疗3个疗程后,观察对比两组患者临床疗效及治疗前后视觉模拟评分量表(VAS)评分、中医症状评分、卵巢异位囊肿大小改善情况。结果:3个疗程结束后,组内比较治疗组和对照组2个疗程后和治疗3个疗程后VAS评分均优于治疗前(P<0.05),组间比较3个疗程结束后,治疗组VAS评分优于对照组(P<0.05);中医症状积分组内比较,与治疗前比,各症状积分较前明显改善(P<0.05);组间比较,两组治疗后小腹冷痛、得热痛减症状改善情况比较,差异有统计学意义(P<0.05),此外治疗组总有效率96.43%(27/28)优于对照组的78.57%(22/28)(P<0.05)且治疗组包块直径明显小于对照组,差异有统计学意义(P<0.05)。结论:铜砭刮痧联合桂香温经止痛胶囊及单纯桂香温经止痛胶囊治疗均能够改善寒凝血瘀型子宫内膜异位症痛经的临床症状,提高患者的生存质量,且铜砭刮痧联合桂香温经止痛胶囊的疗效优于单纯口服桂香温经止痛胶囊,值得临床推广应用。
Objective:To observe the clinical efficacy of scraping combined with Guixiang Wenjing Zhitong Capsule in the treatment of dysmenorrhea of endometriosis(pattern of cold congealing with blood stasis).Methods:56 patients who met the inclusion criteria were randomly divided into the treatment group(n=28)and the control group(n=28).The control group was treated with Guixiang Wenjing Zhitong Capsule;on which basis,the treatment group was also treated with scraping.The scrapping was mainly applied to Governor Vessel,Baliao points,buttocks,Belt Vessel,lower abdomen and groin.The scrapping was once per week.One menstrual cycle was taken as one course of treatment.After three courses of treatment,the clinical efficacy and the improvements of VAS score,TCM symptom scores and ovarian cyst size were compared in the two groups.Results:The VAS scores were significantly improved after two courses and three courses of treatment compared to those before the treatment in the two groups(P<0.05);the improvements in the treatment group was better than those in the control group after three courses of treatment(P<0.05).The TCM symptom scores were significantly improved after the treatment in the two groups(P<0.05);there were statistical differences in the improvements of lower abdominal pain and pain relieve by heat between the two groups(P<0.05).The total effective rate of the treatment group was 96.43%,which was significantly higher than 78.57%of the control group(P<0.05).The diameter of the mass in the treatment group was significantly smaller than that in the control group(P<0.05).Conclusion:Scraping combined with Guixiang Wenjing Zhitong Capsule can improve the clinical symptoms of dysmenorrhea of endometriosis(pattern of cold congealing with blood stasis),it can improve the life quality of the patients.The therapy is superior to single treatment of Guixiang Wenjing Zhitong Capsule.
