摘要
目的探讨早期快速康复综合护理在心脏术后机械通气患者的临床应用及效果评价。方法选取2017年4月至2018年10月在暨南大学附属珠海医院珠海市人民医院心胸外科行心脏手术后气管插管呼吸机辅助呼吸的患者112例,按照术后入住心胸外科监护室顺序分为对照组和试验组,各56例。对照组采取心脏术后常规护理措施,试验组患者予制定个性化的早期快速康复综合护理方案。比较两组患者气管插管一次性成功脱机率、机械通气时间、住院总时间、术后并发生等情况。结果试验组一次性脱机失败的概率由14.30%降至3.70%,患者清醒时间、机械通气时间均缩短约2 d,患者住院时间缩短5 d,试验组不良事件总发生率3.57%,显著低于对照组10.70%,差异均有统计学意义(均P<0.05)。结论早期快速康复综合护理能够缩短机械通气时间,降低呼吸机相关性肺炎发生率、缩短住院天数,增加患者肌力,有效预防ICU获得性衰弱,改善患者预后。
Objective To explore the clinical application and effect evaluation of early rehabilitation comprehensive nursing in patients with mechanical ventilation after cardiac surgery.Methods A total of 112 patients who underwent tracheal intubation and ventilator assisted breathing after cardiac surgery in thoracic surgery department of Zhuhai People's Hospital Affiliated to Jinan University from April 2017 to October 2018 were selected.The control group received routine nursing measures after cardiac surgery,and the experimental group received personalized comprehensive nursing plan for early rapid rehabilitation.The success rate of endotracheal intubation,the duration of mechanical ventilation,the total length of hospital stay,and the postoperative occurrence of endotracheal intubation were compared between the two groups.Results The probability of one-time weaning failure in the experimental group decreased from 14.30%to 3.70%,the patients'waking time and mechanical ventilation time were shortened by about 2 days,and the patients'hospitalization time was shortened by 5 days.The overall incidence of adverse events in the experimental group was 3.57%,which was significantly lower than that in the control group(10.70%),and the differences were statistically significant(P<0.05).Conclusions Early rapid rehabilitation comprehensive nursing can shorten the duration of mechanical ventilation,reduce the incidence of ventilator-associated pneumonia,shorten the length of stay,increase the muscle strength of patients,effectively prevent acquired ICU weakness,and improve the prognosis of patients.
