摘要
目的:研究热奄包联合颈椎保健操治疗神经根型颈椎病(Cervical spondylotic radiculopathy,CSR)患者的临床效果。方法:选取2015年8月-2017年6月濮阳市中医医院CSR患者107例,按随机数字表法分为观察组54例和对照组53例。对照组采用颈椎保健操治疗,观察组在对照组基础上加用热奄包治疗。对比2组临床疗效、症状改善情况、治疗前后颈椎功能障碍指数量表(NDI)评分、视觉模拟量表(VAS)评分、治疗前后血液流变学(血浆比黏度、全血比黏度)状况、复发率。结果:观察组总有效率为92.59%,高于对照组的75.47%(P<0.05)。治疗后与对照组比较,观察组工作、生活及社会能力、主观症状、临床体征评分较高(P<0.05)。治疗后2组颈椎功能及疼痛程度均有不同程度改善,且观察组NDI、VAS评分较对照组低(P<0.05)。治疗后2组血液流变学状况均显著改善,且观察组血浆比黏度、全血比黏度较对照组低(P<0.05)。随访1年,观察组复发率9.30%,较对照组30.00%低(P<0.05)。结论:热奄包联合颈椎保健操治疗CSR患者临床疗效显著,能有效改善患者临床症状及体征,恢复颈椎功能,预防复发。
Objective:To observe the clinical effect of the compress of hot package combined with cervical vertebra exercises for the patients with cervical spondylotic radiculopathy(CSR).Methods:A total of 107 CSR patients in Puyang Hospital of Traditional Chinese Medicine from August 2015 to June 2017 were selected and divided into the observation group with 54 patients and the control group with 53 patients according to the random number table.The control group was treated with cervical vertebra exercises,and the observation group was treated with compress of hot package based on the treatment of the control group.The clinical effect,improvement of symptom,score of Neck Disability Index(NDI)and score of Visual Analogue Scale(VAS)before and after treatment,hemorheological indexes including plasma viscosity and whole blood viscosity before and after treatment,and recurrence rate were compared between the two groups.Results:The total effective rate was 92.59%in the observation group,higher than that of 75.47%in the control group(P<0.05).After treatment,the scores of work,living and social ability,subjective symptoms and clinical signs were higher in the observation group than those in the control group(P<0.05).After treatment,both groups had different degrees of improvement in cervical function and pain,and the NDI and VAS scores in the observation group were lower than those in the control group(P<0.05).The hemorheological indexes in the two groups were significantly improved after treatment,and the plasma viscosity and whole blood viscosity in the observation group were lower than those in the control group(P<0.05).After one-year follow-up,the recurrence rate was 9.30%in the observation group,lower than that of 30.00%in the control group(P<0.05).Conclusion:The therapy of compress of hot package combined with cervical vertebra exercises has significant effect in treating CSR,which can effectively improve the clinical symptoms and signs of patients,restore cervical function and prevent recurrence.
