摘要
近期我院急诊内、外科因静脉注射引起的不良反应事件增加,引发的投诉明显增加,对急诊科静脉注射抗菌药物处方进行调查。目的:为提高我院抗菌药处方质量,规范抗菌药使用,减少耐药菌的产生,安全、有效、经济使用抗菌药。方法:利用我院信息系统调取2019年5月急诊内科、外科处方8556张,抗菌药物处方2580张,静脉注射抗菌药处方1309张,对处方进行病种分布、医生用药习惯、用药频度进行分析,随机抽取1~20号静脉注射抗菌药处方438张进行专项点评及不合理用药点评分析。结果:不合理处方42张,占抽查处方的9.6%(42/438)。结论:我院静脉抗菌药物使用不合理,使用药物抗菌谱与病种分布致病菌有偏差,抗菌药物静脉给药率较高。注射用抗菌药物专项点评有利于发现科室存在的普遍问题,提出干预措施为持续改进提供参考。
Recently,we investigated the prescriptions of antibiotics by intravenous injection in the Emergency Department due to the clearly increasing complaints about adverse reactions caused by intravenous injection in Emergency internal medicine and Emergency surgery.Purpose:We improve the effect of the prescriptions of antibiotics and normalize the usage of antibiotics so that the patients can take the antibiotics safely,effectively and economically and the production of the antibiotics could be reduced.Method:First,We analyse the disease distribution,doctor’s dosage habit and dosage frequency according to 8556 prescriptions extracted from the prescriptions of the Emergency internal medicine and Emergency surgery in May,2019 from the Hospital Information System.There are 2580 prescriptions of antibiotics and 1309 prescriptions of antibiotics by intravenous injection in 8556 prescriptions.Then,we evaluate 438 prescriptions by random sampling from number 1 to number 20 antibiotics by intravenous injection and analyse the unreasonable usage of dosage.At last,after evaluation and analysis,there are 42 prescriptions of the the unreasonable usage of dosage which is 9.6%in the 438 prescriptions.Conclusion:the use of intravenous antibiotics in our hospital is not reasonable,the use of antibiotics spectrum and the distribution of pathogenic bacteria,the rate of intravenous antibiotics is high.Special comments on antibiotics for injection were helpful to find out the common problems in the department and put forward intervention measures to provide reference for continuous improvement.
