轻度与重度糖尿病足患者病原菌分布比较及耐药性分析 被引量:3

Comparison of pathogen distribution and analysis of drug resistance in patients with mild and severe diabetic foot

摘要 目的了解轻度与重度糖尿病足(diabetic foot,DF)患者病原菌分布差异及耐药性情况。方法收集2013年-2018年住院DF患者144例,将Wagner 0级~2级患者设为轻度组,3级~5级患者设为重度组,比较2组病原菌分布及耐药情况。结果144例患者中微生物培养阳性110例,阳性率为76.4%,共培养出微生物160株,其中革兰阳性菌94株,革兰阴性菌59株,真菌7株,单一菌感染68例(61.8%),2种菌感染34例(30.9%),3种及以上菌感染8例(7.3%),排列前4位菌为金黄色葡萄球菌、粪肠球菌、大肠埃希菌、链球菌属,分别占22.50%、15.63%、11.25%、10.63%;重度组粪肠球菌分离率远高于轻度组,差异有统计学意义(χ^2=5.52,P=0.02)。未检出耐万古霉素的革兰阳性菌,未检出耐碳青霉烯类的革兰阴性菌。结论糖尿病足感染以金黄色葡萄球菌分离率最高,重度组粪肠球菌分离率远高于轻度组,铜绿假单胞菌、厌氧菌检出率极低;革兰阳性菌对万古霉素、革兰阴性菌对碳青霉烯类均敏感。 Objective To investigate the distribution and drug resistance of pathogens in patients with mild and severe diabetic foot(DF).Methods A total of 144 patients with DF were enrolled from 2013 to 2018.The patients with grade 0-2 were assigned to the mild group,and the patients with grade 3-5 were set as the severe group.The distribution and drug resistance of the two groups were compared.Results Among the 144 patients,110 were positive for microbial culture,the positive rate was 76.4%.A total of 160 microorganisms were cultured,including 94 Gram-positive strains,59 strains of Gram-negativebacteria,7 strains of fungi,68 cases(61.8%)of single bacteria infection,34 cases(30.9%)of two kinds of bacteria infections,8 cases(7.3%)of three or more kinds of bacteria infections.The top 4 bacteria were Staphylococcus aureus,Enterococcus faecalis,Escherichia coli and Streptococcus,accounting for 22.50%,15.63%,11.25%,and 10.63%,respectively.The segregation rate of Enterococcus faecalis in severe group was much higher than that in mild group,with the difference statistically significant(χ^2=5.52,P=0.02).No gram-positive bacteria resistant to vancomycin and gram-negative bacteria resistant to carbapenems were detected.Conclusion Diabetic foot infection(DFI)has the highest isolation rate of Staphylococcus aureus.The isolation rate of Enterococcus faecalis in severe group is much higher than that in mild group.The detection rates of Pseudomonas aeruginosa and anaerobic bacteria are extremely low;Gram-positive bacteria are sensitive to vancomycin,and Gram-negative bacteria are sensitive to carbapenems.
