摘要
目的运用追踪方法学降低非计划拔管发生率,提高导管护理质量。方法采用个案追踪与系统追踪相结合,运用个案追踪对发生的非计划拔管事件进行逐一追溯,并对日常工作中导管护理质量进行质量控制个案追踪,查找导致非计划拔管的原因及日常工作中存在的不足,从而进行风险控制;运用系统追踪剖析各层面、各关键环节存在的问题,从而制订对策,持续改进评价采用追踪方法学前后6个月非计划拔管发生率。结果运用追踪法后非计划拔管发生率由0.752‰降至0.274‰,实施前后差异有统计学意义(P<0.05)。结论运用追踪方法学进行导管护理管理,有利于提高导管护理质量,降低非计划拔管发生率,确保患者安全。
Objective Reducing the incidence of unplanned extubation using tracking methodology,improve the quality of catheter nursing.Methods Case tracking and systematic tracking were used to trace the unplanned extubation events one by one,and the quality control cases of catheter nursing in daily work were followed up.To find out the causes of unplanned extubation and the shortcomings in daily work,so as to carry out risk control;The problems in various levels and key links were analyzed by systematic tracing,and countermeasures were formulated to continuously improve the incidence of unplanned extubation in 6 months before and after the adoption of tracking methodology.Results The incidence of unplanned extubation decreased from 0.752‰ to 0.274‰ after the use of tracing method,and the difference was statistically significant before and after implementation(P<0.05).Conclusion Using tracing method to manage catheter nursing is beneficial to improve the quality of catheter nursing,reduce the incidence of unplanned extubation,and ensure the safety of patients.
