摘要
目的探讨个案管理模式在预防宫颈癌术后下肢深静脉血栓形成(DVT)中的应用效果。方法选取行手术治疗的宫颈癌患者60例为研究对象,根据入院顺序分为对照组和观察组,各30例。对照组给予常规护理,观察组在对照组的基础上接受个案管理师的指导。评价患者居家康复措施执行情况,比较2组DVT发生率结果观察组患者在穿着弹力袜、踝泵运动、下床活动时间、肌肉等长收缩锻炼、液体摄入方面的依从性优于对照组,差异有统计学意义(P<0.05)。经彩色多普勒超声确诊,对照组4例发生下肢DVT,观察组无1例发生下肢DVT,差异有统计学意义(P<0.05)。结论基于个案管理模式的全程护理能有效提升宫颈癌患者术后康复措施执行的依从性,从而降低DVT的发生率。
Objective To investigate the effect of case management model in prevention of deep vein thrombosis(DVT) after surgery for cervical cancer. Methods Totally 60 patients undergoing surgery for cervical cancer were assigned to the control group and the observation group, with 30 cases in each group. Patients in two groups received conventional care, and those in the observation received rehabilitation interventions based on case management model. The patients’ compliance with rehabilitation intervention at home was evaluated. The incidence of DVT was compared between two groups. Results The compliance rates of wearing stretch socks, Ankle joint pump exercise, off-bed activity, isometric muscular contraction exercise and fluid intake in the observation group were higher than those in the control group(P<0.05). According to the color Doppler ultrasound, 4 cases in the control group and no patient in the observation group had developed DVT, with a significant difference(P<0.05). Conclusion The case management model is effective to enhance the cervical cancer patients’ compliance with rehabilitation interventions after surgery, therefore reducing the incidence of postoperative DVT.
