摘要
目的系统评价穴位按摩对减轻腹腔镜术后胃肠道并发症的效果。方法计算机检索Web of Science、PubMed、ScienceDirect、中国知网(CNKI)、万方医学网(Wanfang Data)、维普(VIP)、中国生物医学文献数据库(CBM),由2名研究者独立按照纳入和排除筛选文献,提取信息,并使用RevMan 5.3软件进行Meta分析。结果共纳入文献17篇,研究对象2 218例。Meta结果示穴位按摩组恶心呕吐[OR=0.29,95%CI(0.22,0.40),P<0.01]、腹胀[OR=0.12,95%CI(0.03,0.40),P<0.01]发生率低于常规护理组,穴位按摩组术后排气时间[SMD=-1.66,95%CI(-2.26,-1.06),P<0.01]、排便时间[SMD=-1.40,95%CI(-2.48,-0.32),P<0.01]、肠鸣音恢复时间[SMD=-7.01,95%CI(-8.16,-5.87),P<0.01]短于常规护理组。结论穴位按摩能有效降低腹腔镜手术患者术后恶心、呕吐、腹胀的发生风险,缩短术后肠鸣音恢复时间、排气时间和排便时间。
Objective To systematically review the effect of acupoint massage in prevention of gastrointestinal complications after laparoscopic surgery in adults. Methods By searching the studies in Web of Science, PubMed, ScienceDirect, CNKI, Wanfang Data, VIP,CBM, two researchers independently select the literature according to the inclusion and exclusion criteria. Data was extracted and a Meta analysis was conducted by RevMan 5.3 software. Results A total of 17 RCTs involving 2218 participants were included. The results of meta-analysis showed that there were statistical differences between acupoint massage and conventional nursing in reducing incidence rate of nausea/vomiting(OR=0.29,95%CI[0.22,0.40],P<0.01) and abdominal distension(OR=0.12,95%CI[0.03,0.40],P<0.01), and in shortening postoperative time of anal exhaust(SMD=-1.66,95%CI[-2.26,-1.06],P<0.01), defecation(SMD=-1.40,95%CI[-2.48,-0.32],P<0.01) and bowel movement(SMD=-7.01,95%CI[-8.16,-5.87],P<0.01). Conclusion The acupoint massage is effective to reduce the risk of nausea/vomiting and abdominal distension in adult patients after laparoscopic surgery. It is effective to improve the postoperative rehabilitation of patients.
