摘要
目的分析918例血液内科需多次输血患者Rh表型及不规则抗体鉴定结果,探讨需多次输血血液病患者输血前检测Rh表型及选择相匹配的Rh表型输血的临床意义。方法采用微柱凝胶检测卡法对918例需多次输血患者进行Rh抗原分型,并采用微柱凝胶检测卡法进行抗体筛选及鉴定。结果918例患者共检出Rh(D)阳性911例,其中911例血标本Rh阳性表型分布规律为CCDee(386例)>CcDEe(332例)>CcDee(79例)>ccDEE(64例)>ccDEe(30例)>CCDEe(10例)>CcDEE(7例)>ccDee(2例)>CCDEE(1例);6例Rh(D)阴性表型分布规律为ccdee(4例)>Ccdee(1例)>CcdEe(1例);1例弱D表型为DCc Ee。D、C、c、E、e抗原频率由高到低分别为D(99.67%)>e(93.96%)>C(91.00%)>c(48.96%)>E(42.92%)。进行Rh配合性输注后共检出不规则抗体6例,其中新产生Rh血型系统相关的不规则抗体仅为1例。结论对于多次输血患者输血选用Rh表型匹配的血液输注,从根本上减少和预防患者Rh血型系统不规则抗体的产生,确保输血的安全性,提高输血疗效,尤其对Rh系统免疫过的患者再次输血具有重要意义。
Objective To analyze the Rh phenotype and irregular antibody identification results of 918 hematology patients undergoing blood transfusion in multiple blood transfusions,and to explore the clinical significance of detecting Rh phenotype and selecting matched Rh phenotype blood transfusion before transfusion.Methods Rh-antigen typing was performed on 918 patients who were to be transfused by microcolumn gel assay.The microcolumn gel assay was used for antibody screening and identification.Results A total of 911 patients with Rh(D)positive were detected in 918 patients.The Rh-positive phenotype of 911 blood samples was CCDee(386 cases)>CcDEe(332 cases)>CcDee(79 cases)>ccDEE(64 cases)>ccDEe(30 cases)>CCDEe(10 cases)>CcDEE(7 cases)>ccDee(2 cases)>CCDEE(1 case);6 cases of Rh(D)negative phenotype distribution law is ccdee(4 cases)>Ccdee(1 case)>Ccd Ee(1 case);1 case of weak D phenotype is DCc Ee.D,C,c,E,e antigen frequencies from high to low are D(99.67%)>e(93.96%)>C(91.00%)>c(48.96%)>E(42.92%).There were 6 irregular antibodies detected after Rh-compatible infusion,and only 1 case of irregular antibodies was related to the newly generated Rh blood group system.Conclusion For the blood transfusion of patients with multiple transfusions,Rh-phenotype-matched blood transfusion is used to fundamentally reduce and prevent the generation of irregular antibodies in the Rh blood group system of patients,and can ensure the safety of blood transfusion,improve the efficacy of blood transfusion,which is especially of great significance for patients who is immune to Rh system.
