摘要
目的:探讨血清CRP(C反应蛋白)在鉴别小儿细菌性肺炎与支原体肺炎中的诊断价值。方法:回顾性收集笔者所在医院65例细菌性肺炎患儿资料为细菌性肺炎组,同期选择65例支原体肺炎患儿资料为支原体肺炎组,检测并比较两组血清CRP水平。根据检测结果分为三个等级,分别为<8、8~40、>40 mg/L,比较两组不同等级CRP水平。通过绘制ROC曲线分析CRP在鉴别支原体肺炎、细菌性肺炎中的诊断效能。结果:细菌性肺炎组CRP为76.36(8.97,325.27)mg/L,显著高于支原体肺炎组的32.60(2.67,78.64)mg/L(P<0.05)。细菌性肺炎组CRP<8 mg/L为0例;CRP 8~40 mg/L为28例,平均(32.33±6.15)mg/L;CRP>40 mg/L为37例,平均(179.17±47.73)mg/L。支原体肺炎组CRP<8 mg/L为29例,平均(5.72±1.49)mg/L;CRP 8~40 mg/L为23例,平均(24.91±5.67)mg/L;CRP>40 mg/L为13例,平均(48.25±9.34)mg/L。细菌性肺炎组在8~40、>40 mg/L中的平均CRP水平均显著高于支原体肺炎组(P<0.05)。ROC曲线下面积为0.880,敏感度为87.7%,特异度为75.40%。结论:细菌性肺炎患儿血清CRP显著高于支原体肺炎患儿,临床可参考CRP水平鉴别细菌性肺炎与支原体肺炎。
Objective:To investigate the value of serum CRP(C-reactive protein)in the differential diagnosis of bacterial pneumonia and mycoplasmal pneumonia in children.Method:The data of 65 children with bacterial pneumonia in the hospital were retrospectively collected as the bacterial pneumonia group,and the data of 65 children with mycoplasmal pneumonia were collected as the mycoplasmal pneumonia group.Serum CRP in the two groups was detected and compared.According to the detection results,it is divided into three grades,respectively<8,8~40,>40 mg/L,and CRP levels of different grades were compared between the two groups.The ROC curve was drawn to analyze diagnostic efficiency of CRP in identifying mycoplasmal pneumonia and bacterial pneumonia.Result:CRP of the bacterial pneumonia group was 76.36(8.97,325.27)mg/L,which was significantly higher than 32.60(2.67,78.64)mg/L of the mycoplasmal pneumonia group(P<0.05).CRP<8 mg/L in the bacterial pneumonia group was 0 cases,and CRP 8~40 mg/L was 28 cases,average(32.33±6.15)mg/L,and CRP>40 mg/L was 37 cases,average(179.17±47.73)mg/L.CRP<8 mg/L in the mycoplasmal pneumonia group was 29 cases,average(5.72±1.49)mg/L,and CRP 8-40 mg/L was 23 cases,average(24.91±5.67)mg/L,and CRP>40 mg/L was 13 cases,average(48.25±9.34)mg/L.The average CRP levels of 8-40 mg/L and>40 mg/L in the bacterial pneumonia group were significantly higher than those of the mycoplasmal pneumonia group(P<0.05).The area under ROC curve was 0.880,the sensitivity was 87.7%,and the specificity was 75.40%.Conclusion:Serum CRP of children with bacterial pneumonia is significantly higher than that of children with mycoplasmal pneumonia.CRP level can be referred to judge bacterial pneumonia and mycoplasmal pneumonia.
