摘要
目的:观察调肠方辅助治疗重症肺炎伴胃肠功能障碍(GIDF)患者的临床疗效及对炎症因子的影响和胃肠功能的保护作用。方法:将62例GIDF患者随机分为观察组和对照组各31例。两组均予常规抗感染及对症治疗。对照组予枸橼酸莫沙必利治疗,观察组予调肠方治疗。两组均连续观察7d。观察两组治疗前后多器官功能障碍综合征(MODS)评分、临床肺部感染评分(CPIS)和胃肠功能障碍评分(GIDS)。检查两组治疗前后胃液PH值、血清降钙素原(PCT)、肿瘤坏死因子(TNF)-α、超敏C反应蛋白(hs-CRP)、白细胞介素(IL)-6、血清胃泌素(GAS)、胃动素(MTL)、二胺氧化酶(DAO)的含量水平。结果:观察组有效率为90.32%,明显高于对照组64.52%,差异具有统计学意义(P<0.05)。观察组治疗后MODS、CPIS、GIDS评分明显低于对照组,差异有统计学意义(P<0.05)。观察组治疗后血清hs-CRP、PCT、TNF-α、IL-6、GAS、DAO含量水平均明显低于对照组,差异有统计学意义(P<0.05)。观察组胃液pH值与MTL血清水平均高于对照组,差异有统计学意义(P<0.05)。结论:调肠方能够辅助治疗重症肺炎伴GIDF患者,其不仅能改善患者胃肠道症状,还能抑制炎症因子分泌,调节胃肠激素及酸碱环境稳定。
Objective To investigate the effect of Tiaochang decoration in treating the severe pneumonia with gastrointestinal dysfunction(GIDF).Methods 62 severe pneumonia patients with GIDF were divided into observation and control group and there were 31 in each.All patients were given the conventional anti-infection and symptomatic treatment.Patients in c control group were treated with mosapride citrate,while others in observation group were treated with Tiaochang decoration.The scores of multiple organ dysfunction syndrome(MODS),clinical pulmonary infection score(CPIS)and gastrointestinal dysfunction score(GIDS)were observed.PH values in the rumen liquid,the serum concentrations of procalcitonin(PCT),tumor necrosis factor(TNF)-α,hypersensitive c-reactive protein(hs-CRP),interleukin(IL)-6,gastrin(GAS),motilin(MTL)and diamine oxidase(DAO)were tested.Results The total effective rate in observation group was 90.32%,which was higher than the control one(64.52%),and there was the difference(P<0.05).The MODS、CPIS、GIDS in observation group was lower than the control group and there were the differences(P<0.05).Moreover,the serum concentrations of hs-CRP,PCT,TNF-α,IL-6,GAS and DAO in observation group were obviously lower than the control group,and there were the differences(P<0.05).In addition,the PH value in the rumen liquid and the serum concentration of MTL in observation group were higher than the control one,and there were the differences(P<0.05).Conclusion:Tiaochang decoration could be the assisted treatment for the severe pneumonia with GIDF.It could not just improve the manifestations,but it could inhibit the secretion of inflammatory factors and regulate the gastrointestinal hormones and the acid-base balance.
