摘要
目的探讨乳果糖灌肠联合门冬氨酸鸟氨酸对肝性脑病患者丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(ALT)及血氨水平的影响。方法选取2016年7月至2019年7月本月中山大学附属第三医院粤东医院的60例肝性脑病患者作为研究对象,根据随机数字表法分为对照组与观察组,每组30例。对照组行注射用门冬氨酸鸟氨酸治疗,观察组在对照组基础上联合乳果糖灌肠治疗,比较两组临床疗效、ALT、AST、血氨水平以及不良反应发生情况。结果观察组治疗有效率(96.67%)高于对照组(76.67%),差异有统计学意义(P<0.05);治疗2周后,观察组血清ALT为(94.97±25.75)U/L、AST为(102.73±30.36)U/L略低于对照组(98.63±27.34)U/L、(104.98±31.21)U/L,差异无统计学意义(P>0.05);治疗2周后,观察组血氨(59.01±17.32)μmol/L低于对照组(97.41±26.49)μmol/L,差异有统计学意义(P<0.05);观察组不良反应发生率20.00%(6/30)略高于对照组13.33%(4/30),但差异无统计学意义(P>0.05)。结论应用乳果糖灌肠联合门冬氨酸鸟氨酸治疗肝性脑病可调节血清AST、ALT水平,且可有效降低患者体内血氨水平,疗效显著,且不增加不良反应发生。
Objective To explore the effects of lactulose enema combined with ornithine aspartate on alanine transaminase(ALT), aspartate transaminase(ALT) and blood ammonia level in patients with hepatic encephalopathy. Methods From July 2016 to July 2019, 60 patients with hepatic encephalopathy in Yuedong Hospital, the Third Affiliated Hospital of Sun Yat-sen University, were selected as the study objects. According to the random number table method, they were divided into control group and observation group, with 30 cases in each group. The control group was treated with aspartate ornithine for injection. The observation group was treated with lactulose enema on the basis of the control group. The clinical effects, ALT, AST, blood ammonia level and adverse reactions were compared between the 2 groups. Results The total effective rate of the observation group(96.67%) was higher than that of the control group(76.67%),(P<0.05). After 2 weeks of treatment, the serum alanine aminotransferase(ALT) was observed in the observation group(94.97±25.75) U/L, aspartate aminotransferase(AST)(102.73±30.36) U/L;After 2 weeks of treatment, the blood ammonia(59.01±17.32) umol/L in the observation group was lower than that in the control group(97.41±26.49) umol/L(P<0.05);the rate of adverse reactions in the observation group was 20.00%(6/30), slightly higher than that of the control group 13.33%(4/30)(P>0.05). Conclusion The application of lactulose enema combined with aspartate ornithine in the treatment of hepatic encephalopathy can regulate the level of AST and ALT in serum, and can effectively reduce the level of blood ammonia in patients, which has a significant effect and does not increase the incidence of adverse reactions.
