摘要
目的采用静息态功能磁共振成像(rs-fMRI)和注意行为学,研究不同性别夜间遗尿症(NE)儿童认知功能差异的脑机制及行为学表现。方法受试儿童为5~11岁,其中NE儿童32例(男15例,女17例),正常儿童35名(男16名,女19名),均行磁共振扫描,采用低频振幅(ALFF)方法进行数据分析,并且分别进行持续性操作测试(CPT)任务,统计正确个数及反应时间。组间差异均采用双样本t检验比较分析。结果功能磁共振:正常男童与女童ALFF比较无明显差异脑区;NE男童与女童ALFF比较有差异的脑区位于右侧额中回(t=4.29)、左侧顶下小叶(t=4.38)、右侧小脑后叶(t=4.46),均为男童ALFF值高于女童(P<0.05),差异有统计学意义。CPT行为学:NE(t=-2.527,P=0.017)与正常(t=-2.511,P=0.017)男童反应时间均低于女童,差异有统计学意义;而正确个数比较差异无统计学意义;NE和正常男童反应时间(t=0.501,P=0.620)和正确个数(t=0.038,P=0.970)差异均无统计学意义;NE和正常女童反应时间(t=0.773,P=0.445)和正确个数(t=0.056,P=0.956)差异亦均无统计学意义。结论 NE女童与男童相比表现出更明显的注意力、执行功能和工作记忆受损。
Objective To study the brain mechanism and behavioral performance of cognitive functional differences between children with nocturnal enuresis(NE) of different genders by resting-state functional magnetic resonance imaging(rs-fMRI) and attentional behavior research. Methods ALL the children participating the test were 5-11 years old including 32 NE children(15 boys and 17 girls) and 35 normal children. They respectively underwent MRI scan and the data were analyzed by the method of amplitude of low frequency fluctuation(ALFF). Continued performance test(CPT) was done and the correct number and response time was recorded. The intergroup variance was analyzed by two sample t-test. Results fMRI:There were no obviously different brain regions of ALFF between normal boys and girls. The brain regions which had obvious differences of ALFF between NE boys and girls were right middle frontal gyrus(t=4.29),left inferior parietal lobule(t=4.38) and right posterior cerbellar lobe(t=4.46). And the group of NE boys were higher than NE girls on all the three regions(P<0.05). CPT:The response time of NE(t=-2.527,P=0.017) and normal(t=-2.511,P=0.017) boys were both lower than those of girls,but there were no statistical differences on the correct number. There were no statistical differences on the response time(t=0.501,P=0.620) and the correct number(t=0.038,P=0.970) between NE and normal boys,and there were no statistical differences on the response time(t=0.773,P=0.445) and the correct number(t=0.056,P=0.956) between NE and normal girls. Conclusion Girls with NE performance more obvious impairment of attention,executive function and working memory than boys.
