MRI联合血清铁蛋白在鉴别IgG阴性肝囊型包虫病和单纯性肝囊肿中的应用研究 被引量:5

Serum Ferritin Combined with MRI in Identifying IgG Negative Application Study of Hepatic Cystic Echinococcosis and Hepatic Cysts

摘要 目的通过研究MRI联合血清铁蛋白(SF)在IgG阴性CE1型肝囊型包虫病(HCE)和肝囊肿中的诊断价值,探索SF对二者的鉴别能力。方法对青海大学附属医院肝胆胰外科2011年8月至2018年8月诊治的80例IgG阴性CE1型HCE和60例单纯性肝囊肿患者的人口统计学、临床及影像学数据进行回顾性分析,统计SF以及与其他多项指标的相关性,通过受试者工作特征曲线(ROC)分析MRI联合SF在鉴别HCE和单纯性肝囊肿中的诊断效能。结果80例IgG阴性CE1型HCE患者平均年龄(37.04±11.07)岁,60例单纯性肝囊肿患者平均年龄(51.38±11.10)岁;HCE和单纯性肝囊肿病灶平均直径约(5.88±1.88)cm、(2.89±1.28)cm;肝囊型包虫病患者SF指标为342.57 ng/ml(114.10~624.96 ng/ml),单纯性肝囊肿SF指标为154.6 ng/ml(90.12~301.48 ng/ml),P<0.05;HCE与单纯性肝囊肿经差异性分析得出,Fe^-、AFP、CA125、CA199、DBIL、TP、TBA、TG、HDL指标存在统计学差异(P<0.05);HCE患者SF指标升高与Fe^-、AFP、AST、ALP、GGT、黄疸指标(TBIL、DBIL、IBIL)、TP、ALB、APTT、病灶长径相关;同时,病灶长径与SF升高相关性最高;而单纯性肝囊肿患者SF升高只与病灶直径相关,且相关系数较肝囊型包虫病小。结论CE1型HCE与单纯性肝囊肿患者SF含量存在明显差异,结合SF指标将提高对CE1型IgG阴性HCE和单纯性肝囊肿的鉴别能力,为临床治疗提供参考信息。 Objective To investigate the diagnostic value of serum ferritin in IgG-negative CE1 hepatic cystic echinococcosis and hepatic cysts,and to explore the ability of serum ferritin to distinguish the two diseases.Methods A retrospective analysis was performed on the demographic,clinical and imaging data of 80 patients with IgG-negative CE1 hepatic cystic echinococcosis and 60 patients with simple hepatic cyst diagnosed and treated by Hepatobiliary and Pancreatic Surgery,Affiliated Hospital of Qinghai University from august 2011 to 2018.The relationship between serum ferritin and other indicators was summarized.The value of MRI combined with serum ferritin in the differential diagnosis of hepatic cystic echinococcosis and simple hepatic cyst was summarized.Results The average age of 80 patients with IgG-negative CE1 hepatic cystic echinococcosis was 37.04±11.07,and the average age of 60 patients with simple hepatic cyst was 51.38±11.10.The average diameter of hepatic cystic echinococcosis and simple hepatic cyst was(5.88±1.88)cm,(2.89±1.28)cm;SF index P<0.05,SF index of liver cystic echinococcosis was 342.57 ng/ml(114.10-624.96 ng/ml),simple liver cyst SF index It was 154.6 ng/ml(90.12-301.48 ng/ml);differention analysis of hepatic cystic echinococcosis and simple hepatic cysts:Fe^-,AFP,CA125,CA199,DBIL,TP,TBA.There were statistical differences in TG and HDL indexes,P<0.05;Elovated SF index in patients with hepatic cystic echinococcosis and Fe^-,AFP,AST,ALP,GGT,jaundice indicators(TBIL,DBIL,IBIL),TP,ALB,APTT.At the same time,the long-path of the lesion was the most correlated with the increase of SF;while the increase of SF in patients with simple hepatic cyst was only related to the diameter of the lesion,and the correlation coefficient was smaller than that of hepatic cystic echinococcosis.Conclusion There is a significant difference in SF content between CE1 type hepatic cystic echinococcosis and simple hepatic cyst.Combined with SF index,it will improve the discriminating ability of CE1 type IgG negative hepatic echinococcosis and simple hepatic cyst,and provide reference for clinical treatment value.
