摘要
目的探讨血管内皮生长因子(VEGF)、血小板应答蛋白l(TSP-l)在乳腺癌中的表达与预后的相关性。方法免疫组织化学染色检测160例乳腺癌组织及癌旁组织VEGF、TSP-1的表达,并分析与患者临床病例指标的关系。结果乳腺癌组织TSP-1表达显著降低、VEGF表达显著增加。TSP-1低表达和VEGF高表达与临床分期高、分化程度差、淋巴结转移相有显著相关性。随访3年,复发率为15.6%0 Spearman等级相关分析,预后复发率与VEGF阳性表达率存在正相关(r=0.459),而与TSP-1阳性表达率存在负相关(r=-0.543)。Logistic回归分析显示TSP-1阳性表达率、VEGF阳性表达率、淋巴结转移、临床分期是影响预后复发的主要独立危险因素。结论乳腺癌组织VEGF高表达、TSP-1低表达,且与临床主要特征有显著的相关性;VEGF高表达、TSP低表达乳腺癌患者的预后复发率高。
Objective To investigate the correlations between the expression of vascular endothelial growth factor(VEGF)and thrombosp ondin 1(TSP-1)in breast can cer and the prog no sis.Methods Immuno histochemical staining was used to detect the expression of VEGF and TSP-1 in 160 cases of breast cancer tissues and adjace nt tissues,and the relation ships betwee n them were an alyzed.Results The expressi on of TSP-1 sign ificantly decreased and the expression of VEGF significantly increased in breast cancer tissues.Low expression of TSP-1 and high expression of VEGF were significantly associated with high clinical stage,poor differentiation,and lymph node metastasis.After 3 years of follow-up,the recurrenee rate was 15.6%.Spearman rank correlation analysis showed that there was a positive correlation between the prog nosis recurre nee rate and the positive expressi on rate of VEGF(r=0.459),but n egatively correlated with the positive expression rate of TSP-1(r=-0.543).Logistic regression analysis showed that TSP-1 positive expression rate,VEGF positive expressi on rate,lymph node metastasis and clinical stage were the main in depe ndent risk factors for prog nosis and recurrence.Conclusion The high expression of VEGF and the low expression of TSP-1 in breast cancer tissues are significantly correlated with the main clinical features.The recurrenee rate of patients with high expression of VEGF and low expression of TSP is high.
