摘要
目的探讨T淋巴细胞亚群、基质金属蛋白酶9(MMP9)、基质金属蛋白酶2(MMP2)在原发性肺癌(PLC)中的表达情况和临床意义。方法选取89例PLC患者作为PLC组,选取同期体检的30例健康者作为对照组。采用流式细胞仪测定外周血中T淋巴细胞亚群水平,采用酶联免疫吸附试验法(ELISA)测定血清中的MMP9、MMP2水平,分析上述指标与PLC患者病理特征的关系及上述指标对PLC是否发生远处转移的预测价值。结果PLC组患者外周血中的CD3-CD19+、CD8+CD28+水平均明显低于对照组受试者,血清中的MMP9、MMP2水平均明显高于对照组受试者(P﹤0.01)。不同病理类型PLC患者外周血中T淋巴细胞亚群及血清中的MMP9、MMP2水平比较,差异均无统计学意义(P﹥0.05)。不同N分期PLC患者血清中的MMP9、MMP2水平比较,差异均无统计学意义(P﹥0.05);Ⅲ~Ⅳ期、低分化、N1~3期、M1期的PLC患者外周血中CD3-CD19+、CD8+CD28+水平均明显低于Ⅰ~Ⅱ期、中高分化、N0期、M0期患者(P﹤0.01);Ⅲ~Ⅳ期、低分化、M1期的PLC患者血清中MMP9、MMP2水平均明显高于Ⅰ~Ⅱ期、中高分化、M0期患者(P﹤0.01)。相关性分析结果显示,PLC患者外周血中CD3-CD19+、CD8+CD28+水平与血清中MMP9、MMP2水平均呈负相关(P﹤0.05)。ROC曲线分析结果显示,MMP9水平对PLC远处转移的预测价值最高,其次为CD3-CD19+。结论PLC患者外周血中的CD3-CD19+、CD8+CD28+水平均较低,血清中的MMP9、MMP2水平均较高,其均参与PLC的发病和进展过程,且与临床分期、肿瘤分化程度及是否发生远处转移有关,亦可能协同参与PLC细胞的浸润、转移过程。
Objective To investigate the expression of T lymphocyte subsets,matrix metalloproteinase 9(MMP9)and matrix metalloproteinase 2(MMP2)in primary lung cancer(PLC)and to determine their clinical significance.Method A total of 89 PLC patients were enrolled as PLC group,while 30 healthy subjects who underwent physical examination in the same period were included as control group.The levels of T lymphocyte subsets in peripheral blood were measured by flow cytometry.Enzyme-linked immunosorbent assay(ELISA)was applied to determine the levels of serum MMP9 and MMP2.The relationship between the above indicators and pathological parameters of PLC was analyzed,besides,the predictive value of these indicators for PLC distant metastasis was assessed.Result The levels of CD3-CD19+,CD8+CD28+in PLC group were significantly lower than those in control group,the levels of MMP9,MMP2 in PLC group were higher than those in control group(P<0.01).Among patients with different pathological types of PLC,the levels of lymphocyte subsets in peripheral blood,and levels of serum MMP9 and MMP2 were similar(P>0.05).Additionally,there was no significant difference between the levels of MMP9 and MMP2 in the serum of PLC patients with different N stages(P>0.05);the levels of CD3-CD19+and CD8+CD28+in peripheral blood in PLC patients with clinical stage III-IV,poor differentiation,lymph node metastasis stage N1-3 and distant metastasis stage M1 were lower than those with clinical stage I-II,moderate to high differentiation,lymph node metastasis stage N0 and distant metastasis stage M0(P<0.01).The levels of serum MMP9 and MMP2 in PLC patients with clinical stage III-IV,poor differentiation and distant metastasis stage M1 were significantly higher than those of PLC patients with clinical stage I^II,moderate to high differentiation and distant metastasis stage M0(P<0.01).Correlation analysis revealed that,CD3-CD19+and CD8+CD28+in peripheral blood were negatively correlated with MMP9 and MMP2 in the serum of PLC patients(P<0.05).Through receiver operating characteristic(ROC)analysis,it was found that MMP9 showed the highest predictive value for distant metastasis of PLC,followed by CD3-CD19+.Conclusion T lymphocyte subsets CD3-CD19+and CD8+CD28+in peripheral blood are relatively low,while serum MMP9 and MMP2 are obviously increased in PLC patients.They are all involved in the occurrence and progression of PLC,and are related to clinical stage,tumor differentiation and presence of distant metastasis,it is speculated that they may synergistically participate in the process of PLC cancer cell invasion and metastasis.
