重复经颅磁刺激治疗血管性痴呆对患者血清NSE水平及认知功能的影响 被引量:14

Effects of repetitive transcranial magnetic stimulation on serum NSE level and cognitive function in vascular dementia patients

摘要 目的探讨重复经颅磁刺激(rTMS)治疗血管性痴呆(VaD)对患者血清神经元特异性烯醇化酶(NSE)水平及认知功能的影响。方法将该院2017年4月至2019年3月收治的96例VaD患者按照随机数字表法分为对照组和治疗组,每组48例。对照组进行基础治疗的同时应用丁苯酞治疗,治疗组在对照组基础上应用rTMS治疗。比较两组患者的临床疗效、认知功能、智力、日常生活自理能力及血清NSE、血管紧张素Ⅱ(Ang-Ⅱ)水平。结果治疗后,两组患者简易智能精神状态量表(MMSE)、改良谷川简易智能修订量表(HDS-R)及Barthel指数评定量表(BI)评分均高于治疗前(P<0.05),且治疗组均高于对照组(P<0.05);治疗后,两组患者血清NSE、Ang-Ⅱ水平与治疗前比较均下降(P<0.05),且治疗组水平低于对照组(P<0.05)。结论对VaD患者实施rTMS治疗,临床疗效显著,可明显改善患者认知功能及智力,提高患者生活自理能力,并有效降低血清NSE、Ang-Ⅱ水平。 Objective To investigate the effects of repetitive transcranial magnetic stimulation(rTMS)on serum neuron-specific enolase(NSE)level and cognitive function in patients with vascular dementia(VaD).Methods A total of 96 patients with VaD admitted to the hospital from April 2017 to March 2019 were divided into control group and treatment group according to random number table method,48 cases in each group.The control group was treated with butylphthalide at the same time of basic treatment,and the treatment group was treated with rTMS on the basis of the control group.The clinical efficacy,cognitive function,intelligence,self-care ability of daily life,serum NSE and angiotensinⅡ(Ang-Ⅱ)levels were compared between the two groups.Results After treatment,the scores of mini-mental state examination(MMSE),improved hasegawa dementia scale-revised(HDS-R)and barthel index(BI)in both groups were higher than those before treatment(P<0.05),and the treatment group were higher than the control group(P<0.05).After treatment,the levels of serum NSE and Ang-Ⅱin the two groups were lower than those before treatment(P<0.05),and the treatment group were lower than the control group(P<0.05).Conclusion rTMS treatment for VaD patients has significant clinical effect,it can significantly improve the cognitive function and intelligence of patients,improve the self-care ability of patients,and effectively reduce the levels of serum NSE and Ang-Ⅱ.
