摘要
目的:观察慢性心力衰竭(CHF)中医证型与Ca-125、BNP值及心功能分级的相关性。方法:选取2017年1月至2019年6月江西中医药大学附属医院心血管内科收治的CHF患者150例,由3名主任中医师进行辨证分型,记录个证型患者心功能分级、BNP及Ca-125表达水平,并进行统计分析。结果:CHF中医证型与心功能等级的判定呈显著相关(r=0.750,P<0.01),其中气阴两虚证主要以心功能Ⅱ级为主,气虚血瘀证以心功能Ⅱ、Ⅲ级为主,痰瘀互阻证以心功能Ⅲ为主,阳虚水泛证以心功能Ⅲ、Ⅳ级为主,阳虚喘脱证则以心功能Ⅳ级为主,CHF中医证型严重程度与BNP值呈正相关(r=0.437,P<0.01),各证型间差异具有统计意义(P<0.01),CHF中医证型严重程度与Ca-125水平具有相关性(r=0.354,P<0.01),气阴两虚证与阳虚水泛证、阳虚喘脱证比较具有显著差异(P<0.01),气虚血瘀证与阳虚水泛证、阳虚喘脱证相比具有显著差异(P<0.01)。结论:CHF中医证型严重程度与心功能分级呈显著正相关;BNP值可以作为CHF中医辨证分型的客观化参考指标;但从本研究数据来看,Ca-125值与CHF中医证型虽具有一定相关性,但相关性不显著,Ca-125作为CHF中医辨证分型的客观化指标还有待进一步研究。
Objective To observe the correlation between the TCM syndrome types of chronic heart failure(CHF)and CA-125,BNP value and cardiac function grading.Methods 150 CHF patients were selected from the cardiovascular department of the Affiliated Hospital of Jiangxi University of traditional Chinese medicine from January 2017 to June 2019.Three chief TCM doctors were responsible for syndrome differentiation and typing,recording the cardiac function grading,BNP and CA-125 expression level of individual syndrome types,and making statistics.Analysis.Results There was a significant correlation between CHF TCM syndrome type and heart function grade(r=0.750,P<0.01),in which Qi Yin deficiency syndrome was mainly of heart function gradeⅡ,Qi deficiency and blood stasis syndrome was mainly of heart function gradeⅡ,Ⅲ,phlegm stasis syndrome was mainly of heart function gradeⅢ,Yang deficiency and water flooding syndrome was mainly of heart function gradeⅢ,Ⅳ,Yang deficiency and dyspnea syndrome was mainly of heart function gradeⅣ,CHF TCM syndrome type was serious.There was a positive correlation between degree and BNP(r=0.437,P<0.01).There was a statistical significance between the different syndromes(P<0.01).There was a correlation between the severity of CHF syndromes and the level of CA-125(r=0.354,P<0.01).There was a significant difference between the deficiency of Qi and Yin syndromes and the deficiency of Yang,the deficiency of water and the deficiency of Yang,and the deficiency of Yang,and the deficiency of blood stasis syndromes(P<0.01).Conclusion There is a significant positive correlation between the severity of CHF syndrome types and the classification of heart function;BNP value can be used as an objective reference index for CHF syndrome differentiation;however,from the data of this study,although CA-125 value has a certain correlation with CHF syndrome types,the correlation is not significant,and CA-125 as an objective index for CHF syndrome differentiation needs further study.
