核酸检测与ELISA检测在无偿献血血液标本乙型肝炎病毒筛查中的应用效果比较 被引量:4

Comparison of Nucleic Acid Detection and ELISA Detection in Hepatitis B Virus( HBV) Screening of Blood Samples of Unpaid Blood Donation

摘要 目的比较核酸检测与ELISA检测在无偿献血血液标本乙型肝炎病毒(HBV)筛查中的应用效果。方法选择2018年1月—12月期间抚州市23960份无偿献血血液标本,采用核酸检测法及ELISA检测法进行HBV筛查。计算两种检测在HBV筛查中的敏感性、特异性及准确性。结果核酸检测在HBV筛查中的敏感性、特异性及准确性分别为97.50%、99.97%、99.97%,均高于ELISA检测的95.00%、96.13%、96.13%,差异有统计学意义(P<0.05);23960份标本中出现窗口期标本29份,检出率为0.12%,核酸检测的窗口期为(22.53±4.16)d,低于ELISA检测的(35.87±5.54)d,差异有统计学意义(P<0.05)。结论核酸检测与ELISA检测在无偿献血血液标本HBV筛查中均有较高的应用价值,但核酸检测特异性及准确性更高,可缩短病毒窗口期,降低漏诊风险。 Objective To compare the effects of nucleic acid detection and ELISA on hepatitis B virus(HBV)screening in blood samples from unpaid blood donation.Methods A total of 23960 blood donation blood samples from Fuzhou City,Jiangxi Province from January 2018 to December 2018 were selected.HBV screening was performed by nucleic acid detection and ELISA.The sensitivity,specificity,and accuracy of the two assays in HBV screening were calculated.Results The sensitivity,specificity and accuracy of nucleic acid detection in HBV screening were 97.50%,99.97%,and 99.97%,respectively,which were higher than 95.00%,96.13%,and 96.13%of ELISA.The difference was statistically significant(P<0.05);29 specimens appeared in 23960 specimens,the detection rate was 0.12%,the window period of nucleic acid detection was(22.53±4.16)d,which was lower than that of ELISA(35.87±5.54)d.Academic significance(P<0.05).Conclusion Nucleic acid detection and ELISA detection have high application value in HBV screening of blood donation blood samples,but the specificity and accuracy of nucleic acid detection are higher,which can shorten the window period of virus and reduce the risk of missed diagnosis.
