基于氧化应激与Treg细胞探讨补乌煎剂联合308准分子治疗进展期白癜风的作用机制 被引量:15

摘要 目的:观察中药补乌煎剂联合308准分子光治疗进展期白癜风的临床疗效,检测治疗前后外周血氧化-抗氧化指标及CD4^+CD25^+CD127^LOW/-调节性T细胞(Treg)的水平,探讨其治疗的作用机制。方法:根据进展期白癜风患者的纳入标准选取患者65例,随机分为观察组(32例)和对照组(33例)。对照组采用308准分子光治疗,观察组采用补乌煎剂联合308准分子光治疗,疗程为3个月,两组患者治疗前后分别采用生化法检测丙二醛(MDA)、过氧化氢酶(CAT)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)水平;流式细胞仪检测外周血淋巴细胞中CD4^+CD25^+CD127^LOW/-Treg细胞占比。结果:临床疗效比较,与对照组比较差异有统计学意义(χ^2=6.292,P<0.05);两组治疗后患者血清SOD、CAT、GSH-Px水平显著提高,MAD水平显著降低,观察组优于对照组(P<0.01);两组治疗后外周血淋巴细胞中CD4^+CD25^+CD127^LOW/-Treg在CD4+细胞中的比例升高,观察组优于对照组(P<0.01);结论:中药补乌煎剂联合308准分子光治疗能控制进展期白癜风的进展,且有很好的复色作用,其机制可能通过调节机体的氧化-抗氧化失衡从而上调CD4^+CD25^+CD127^LOW/-Treg细胞水平从而减轻对黑素细胞的损伤发挥对该病的治疗作用。 Objective:To observe the clinical efficacy of Buwu decoction combined with the 308 excimer light in the treatment of advanced vitiligo,to investigate its antioxidant index of peripheral blood oxidation and the levels of CD4^+CD25^+CD127^LOW/-regulatory T(Treg) cells,and to explore its action mechanisms.Methods:Sixty-five patients were randomly divided into the observation group(32 cases) and the control group(33 cases) in accordance with the diagnosis criteria of those diagnosed with advanced vitiligo.The 308 excimer light was used in the control group,while Buwu decoction combined with the 308 excimer light was used in the observation group.The duration of the treatment was three months.The levels of MDA,CAT,SOD and GSH-Px in serum were biochemically tested before and after the treatment.The proportion of CD4^+CD25^+CD127^LOW/-T cells in the peripheral blood lymphocytes was tested by flow cytometry.Results:In comparison with the control group,the clinical efficacy of the observation group was statistically different(χ^2=6.292,P<0.05).After the treatment,the decrease of MDA and the increase of CAT,SOD and GSH-Px in the serum of the observation group were more obvious than that of the control group(P<0.01).Both groups showed the increases of the proportion of CD4^+CD25^+CD127^LOW/-T cells in peripheral blood lymphocytes,however,the observation group demonstrated a better result than the control group(P<0.01).Conclusion:Buwu decoction combined with the 308 excimer light could not only control the development of advanced vitiligo,but also had a quite good repigmentation effect.It could regulate the imbalance of oxidation-antioxidation in the body.Subsequently,it could thus up-regulate the level of CD4^+CD25^+CD127^LOW/-T cells,reduce the damage of melanocytes and play a therapeutic role for advanced vitiligo.
