摘要
目的探讨脊柱手术后患者谵妄发生的相关因素及护理对策。方法选取2017年6月至2019年6月我院行脊柱手术患者210例为研究对象,采用回顾性调查方式收集患者相关资料,采用单因素和多因素logistic回归分析,确定脊柱手术后患者谵妄的独立影响因素,提出针对性护理防范对策。结果210例脊柱手术后患者中谵妄发生38例(18.10%)。单因素结果分析显示,年龄、体质量(BMI)、冠心病、手术时间、麻醉方式、是否气管插管、是否发热、是否出现低氧血症、氯离子、白蛋白及红细胞压积为脊柱手术后患者谵妄的相关影响因素(P<0.05)。多因素logistic回归分析显示,高龄、合并慢性疾病、发热及低氧血症均为脊柱手术后患者谵妄的独立危险因素。结论影响患者术后谵妄发生的因素较多,应加强围术期护理干预,及时纠正和发现相关影响因素,降低患者术后谵妄的发生概率。
Objective To explore the factors related to delirium and nursing countermeasures in patients after spinal surgery.Methods To select 210 cases of patients undergoing spinal surgery in our hospital from June 2017 to June 2019 as the study objects.Patients'relevant data were collected by using retrospective survey methods.Univariate and multivariate logistic regression analysis were used to determine the independence influencing factors of delirium of patients after spinal surgery,and put forward countermeasures for targeted nursing prevention.Results There were 38 cases(18.10%)of delirium that occurred in 210 cases of patients after spinal surgery.Univariate analysis showed that the age,body mass index(BMI),coronary heart disease,operation time,anesthesia method,whether or not tracheal intubation,whether or not fever,whether or not hypoxemia,chloride ion,albumin and hematocrit,were the related influencing factors of delirium after spinal surgery(P<0.05).Multivariate logistic regression analysis showed that the advanced age,concomitant chronic diseases,fever,and hypoxemia were the independent risk factors for delirium after spinal surgery.Conclusion There were many factors affecting the incidence of postoperative delirium in patients.The perioperative nursing intervention should be strengthened to timely correct and find the relevant influencing factors to reduce the incidence of postoperative delirium in patients.
