摘要
目的探讨接纳与承诺疗法对膀胱癌术后灌注化疗出院患者恐惧疾病进展的影响。方法将97例膀胱癌术后行膀胱灌注化疗患者按住院时间分为对照组47例和观察组50例,对照组接受常规的健康教育和出院随访,住院期间行接纳与承诺疗法干预3次;观察组在对照组基础上,于出院后2~3个月对患者行接纳与承诺疗法干预3~4次。采用癌症患者恐惧疾病进展简化量表对两组患者恐惧疾病进展进行测评。结果两组患者恐惧疾病进展评分的组间效应、时间效应、交互效应差异有统计学意义(P<0.05);出院3个月和出院6个月观察组患者恐惧疾病进展评分低于对照组(P<0.001)。结论应用接纳与承诺疗法的延续性护理干预有利于降低膀胱癌术后灌注化疗出院患者癌症复发恐惧情绪。
Objective To investigate the effects of acceptance and commitment therapy on the progression of pathophobia in patients with postoperative chemotherapy for bladder cancer.Methods A total of 97 patients with bladder cancer after intravesical instillation chemotherapy were divided into control group(n=47)and observation group(n=50).In the control group routine health education and discharge follow-up were conducted,with interventions and commitment therapy 3 times during the hospitalization.In the observation group,on the basis of treatment as in the control group,intervention and commitment therapy intervention were done 3 to 4 times for 2 to 3 months after discharge.The pathophobia progression simplification scale for cancer patients was used for evaluation.Result The scores of fear disease progression in the two groups were statistically significantly different in between-group effect,time effect and interaction effect(P<0.05).2 months after discharge,the scores of pathophobia progression in the two groups were significantly lower than those at the time of discharge.Conclusion The implementation of continuous care intervention with acceptance of commitment therapy is beneficial for their reducing pathophobia in patients undergoing postoperative chemotherapy for bladder cancer.
