摘要
目的分析某医院门诊非甾体抗炎药(NSAIDs)的使用情况,为临床合理用药提供参考。方法收集河南省洛阳正骨医院2015—2017年口服NSAIDs的相关用药信息,对药物种类、销售金额、用药频度(DDDs)、限定日费用(DDC)和排序比进行统计分析,并抽取100张/月NSAIDs处方进行用药合理性分析。结果NSAIDs总销售金额连续3年呈递增趋势,选择性新型的环氧合酶2(COX⁃2)抑制剂中艾瑞昔布片3年均稳居销售金额排名的第1位,塞来昔布胶囊3年均稳居DDDs排名的第1位,艾瑞昔布片的DDC最大,美洛昔康胶囊的DDC最小;非选择性NSAIDs中醋氯芬酸缓释片连续3年稳居销售金额和DDDs排名的第1位,氨酚双氢可待因片的DDC最大,双氯芬酸钠缓释胶囊的DDC最小,总体同步性好。NSAIDs处方不合理用药类型主要为用法与用量不适宜、联合用药不适宜、重复给药。结论该院NSAIDs用药结构日趋合理,选择性COX⁃2抑制剂的使用频率不断增大,不合理用药情况仍存在,临床应用应根据病人情况选择合适的药物,提高用药安全水平。
Objective To investigate the utilization of oral non⁃steroidal anti⁃inflammatory drugs(NSAIDs)in our hospital and pro⁃vide reference for clinical rational drug use.Methods The relevant medication information of oral NSAIDs of Henan Luoyang Or⁃thopedic⁃Traumatological Hospital from 2015—2017 was collected,and the drug varieties,sales amount,defined daily doses(DDDs),average daily cost(DDC),and sorting ratio were analyzed statistically,and 100 prescriptions with NSAIDs were extracted for drug rationality analysis monthly.Results Consumption sum of oral NSAIDs showed a rising trend for three consecutive years.Consumption sum of imrecoxib tablets in the new cyclooxygenase⁃2(COX⁃2)selective inhibitors ranked the first for three consecu⁃tive years,DDDs of celecoxib capsules for the COX⁃2 selective inhibitors ranked the first for three consecutive years,DDC of imre⁃coxib tablets ranked the first,while DDC of meloxicam capsules ranked the last.Consumption sum and DDDs of aceclofenac sus⁃tained⁃release tablets for the non⁃selective NSAIDs ranked the first for three consecutive years,DDC of aminophenol dihydroco⁃deine tablets ranked the first,while DDC of diclofenac sodium sustained release capsules ranked the last.The main irrational drug application included improper dosage,improper usage combination and drug repetition.Conclusion The medication structure of NSAIDs has become more and more rationalized in our hospital,while the market share of COX⁃2 selective inhibitors is continuous⁃ly expending.The irrational use of drugs still exists.In the application of NSAIDs,the appropriate drugs should be selected accord⁃ing to the patient’s condition to improve the safety level of medication.
