摘要
目的比较人工全膝关节置换术(TKA)术后是否进行闭式引流对患者早期预后的影响。方法将自2015-10—2018-03因膝关节退行性骨关节炎行TKA的87例患者进行随机分组,观察组43例,对照组44例。所有患者均进行初次单侧TKA。记录手术前后血红蛋白量降低值、视觉模拟评分法(VAS)评分差值、大腿周径的差值及美国膝关节协会评分量表(KSS)评分差值,住院时间、术后是否进行输血治疗及术后是否形成血栓。结果87例均获得随访,随访时间3.5~6.0(4.71±1.46)个月。观察组和对照组术后下肢深静脉血栓形成的发生率比较差异无统计学意义(P>0.05)。观察组血红蛋白降低值及术后输血治疗的发生率小于对照组,差异有统计学意义(P<0.05)。结论单纯初次TKA术后常规闭式引流并没有使患者获益,闭式引流可能增加术后出血和输血治疗的概率。
Objective To compare the effects of closed suction drainage on early prognosis in patients after total knee arthroplasty(TKA).Methods From October 2015 to March 2018,87 patients who underwent TKA for degenerative osteoarthritis were randomly assigned,43 patients in the observation group and 44 patients in the control group.All patients underwent initial unilateral TKA.The reduction of hemoglobin volume before and after surgery,the difference of Visual Analogue Scale(VAS)score,the difference of thigh circumference and the American Knee Society Knee Score(KSS)score,the length of hospital stay,whether blood transfusion was performed after surgery,and whether thrombosis occurred after surgery were recorded.Results A total of 87 patients were followed up for 3.5-6.0 months with an average of(4.71±1.46)months.There was no significant difference in the incidence of deep venous thrombosis between the observation group and the control group(P>0.05).The hemoglobin reduction value and the incidence of postoperative blood transfusion in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The patients do not benefit from conventional closed suction drainage after primary TKA,and closed suction drainage may increase the risk of postoperative bleeding and transfusion therapy.
