摘要
目的间断性冷疗在老年膝骨关节炎患者全膝关节置换术后的应用效果。方法将符合入选标准的患者按随机数表法分为无冷疗组(A组)和间断性冷疗组(B组),各45例。对比两组术后膝关节疼痛程度、肿胀程度、引流管引流量、膝关节功能评分(HSS)、术后住院时间及术后并发症发生率。结果两组患者术后6、12、24、48、72 h疼痛程度、肿胀程度差异均有统计学意义(P<0.05);两组患者术后6、12、24、48 h引流量比较,差异均有统计学意义(P<0.05);两组术后出院时、出院后1个月HSS评分比较,差异均有统计学意义(P<0.05);两组术后住院时间比较,差异有统计学意义(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论间断性冷疗应用于老年膝骨关节炎患者全膝关节置换术后,可降低术后膝关节疼痛、肿胀程度,减少术后引流量,减少术后住院时间,改善膝关节功能,但不会增加术后并发症发生率,从而促进患者术后康复。
Objective To investigate the effect of intermittent cold therapy on rehabilitation of patients with knee osteoarthritis after primary unilateral total knee arthroplasty.Methods Patients who met the criteria for admission were randomly divided into two groups:cold therapy-free group(group A)and intermittent cold therapy group(group B),45 cases in each group.The degree of knee pain,swelling,drainage volume,knee function score(HSS),postoperative hospitalization days and postoperative complications were compared between the two groups.Results There were significant differences in pain degree and swelling degree at 6,12,24,48,72 hours after operation(P<0.05).The drainage volume of patients in two groups at 6,12,24,48 hours after operation,and the differences were statistically significant(P<0.05).There were significant differences in HSS scores at the time of discharge after operation and one month after discharge(P<0.05).The difference of postoperative hospital stay between two groups was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications(P>0.05).Conclusion The application of intermittent cold therapy in elderly patients with knee osteoarthritis after total knee arthroplasty can reduce postoperative knee pain and swelling,reduce postoperative drainage,reduce postoperative hospital stay and improve knee function,but does not increase the incidence of postoperative complications,thus promoting postoperative rehabilitation.
