炎症性肠病患者血清IgM、IgG、IgA水平与肠道菌群的相关性 被引量:13

The Correlations Between Serum IgM,IgG and IgA Levels and Intestinal Flora in Patients with Inflammatory Bowel Disease

摘要 目的探讨炎症性肠病(IBD)患者血清免疫球蛋白M(IgM)、免疫球蛋白G(IgG)、免疫球蛋白A(IgA)水平与肠道菌群的相关性。方法选取2016年1月至2019年1月期间我院收治的120例IBD患者作为研究对象。其中活动期78例(活动组),缓解期42例(缓解组),选取同期到我院进行健康体检者60例作为对照组。比较各组间肠道菌群、血清IgM、IgG、IgA水平,并进行Spearman相关性分析和Logistic回归分析。结果相比于对照组,活动组和缓解组肠杆菌、肠球菌增多,双歧杆菌、乳酸杆菌减少(P<0.05);相比于缓解组,活动组肠杆菌、肠球菌增多,双歧杆菌、乳酸杆菌减少(P<0.05)。相比于缓解组和对照组,活动组血清IgM、IgG、IgA水平升高(P<0.05)。经过Spearman相关性分析,IBD患者IgM、IgG、IgA与双歧杆菌、乳酸杆菌、B/E值均呈负相关(P<0.05),与肠杆菌、肠球菌均呈正相关(P<0.05)。经Logistic回归分析,IgM、IgG、IgA是IBD患者B/E值的影响因素。结论 IBD患者存在明显的肠道菌群失调,且影响机体免疫球蛋白的表达。 Objective To investigate the relationship between serum immunoglobulin M(IgM),immunoglobulin MG(IgG),immunoglobulin A(IgA)and intestinal flora in patients with inflammatory bowel disease(IBD).Methods From January,2016 to January,2019,120 patients with IBD were selected as the study subjects.Among them,78 cases were in active stage(the active group)and 42 cases were in remission stage(the remission group).60 cases of healthy people from physical examination in our hospital at the same time were selected as the control group.The levels of intestinal flora and serum IgM,IgG,and IgA were compared among groups,and Spearman correlation analysis and logistic regression analysis were also performed.Results Compared with the control group,Enterobacteriaceae,Enterococcus and Lactobacillus in the active group and remission group were higher,Bifidobacillus and Lactobacillus were lower(P<0.05),and Enterobacteriaceae,Enterococcus,Lactobacillus and Lactobacillus in the active group were higher than those in the remission group(P<0.05).Compared with the remission group and control group,the level of serum IgM,IgG,and IgA in the active group were higher than that in the remission group and control group.Spearman correlation analysis showed that IgM,IgG,and IgA were negatively correlated with Bifidobacterium,Lactobacillus and B/E,and positively correlated with Enterobacteriaceae and Enterococcus(P<0.05).Logistic regression analysis showed that IgM,IgG,and IgA were the influencing factors of B/E value in IBD patients.Conclusion There is an obvious imbalance of intestinal flora in patients with IBD,and the expression of immunoglobulin is affected.
