摘要
目的:探讨血清中肿瘤标志物糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、糖类抗原15-3(CA15-3)、癌胚抗原(CEA)、甲胎蛋白(AFP)在2型糖尿病病人中糖脂代谢的相关性及临床意义。方法:随机抽取本院内分泌科210例2型糖尿病患者作为糖尿病组,同时抽取200例体检者作为对照组,检测FPG、HbA1c、TC、HDL-C、LDL-C、TG、CA19-9、CEA、CA15-3、CA125、AFP指标。结果:糖尿病组FPG、HbA1c、TG、CEA、CA19-9超检测正常值人数高于对照组(P<0.01);同时糖尿病组检测指标FPG、HbA1c、TG、CEA、CA19-9高于对照组(P<0.01),而HDL-C、LDL-C低于对照组(P<0.05);Pearson相关分析显示CA19-9与FPG、HbA1c、TC、TG、LDL-C之间呈正相关性(P<0.05);CEA与FPG、HbA1c之间呈正相关性(P<0.05);而AFP、CA125、CA15-3与各因素无相关性。结论:糖脂代谢可以影响血清肿瘤标志物CA19-9和CEA水平,2型糖尿病患者CA19-9和CEA的升高,应结合FPG、HbA1c、TC、TG等因素综合判断。
Objective:To investigate the correlation and clinical significance of serum tumor markers CA19-9,CA125,CA15-3,CEA and AFP in patients with type 2 diabetes mellitus.Methods:210 patients with type 2 diabetes treated in our endocrinology department were randomly recruited as the diabetes group,and another 200 subjects undergone physical examination were included as controls.Indicators,including FPG,HbA1 c,TC,HDL-C,LDL-C,TG,CA19-9,CEA,CA15-3,CA125 and AFP,were detected in all participants.Results:The number of normal values of FPG,HbA1 c,TG,CEA and CA19-9 was significantly higher in the diabetic group than in the control group(P<0.01).Still,higher values of FPG,HbA1 c,TG,CEA and CA19-9 were found in the diabetes group,yet HDL-C and LDL-C were lower compared to the control group(both P<0.05).Pearson correlation analysis showed that CA19-9 and FPG,HbA1 c,TC,TG and LDL-C were positively correlated(P<0.05),whereas CEA and FPG and HbA1 c were negatively(P<0.05).There was no correlation between AFP,CA125 and CA15-3.Conclusion:Glucose and lipid metabolism can affect the serum tumor marker CA19-9 and CEA levels.Elevated CA19-9 and CEA are found in type 2 diabetes patients,suggesting that diagnosis should be combined with FPG,HbA1 c,TC,TG and other factors.
