摘要
目的 分析甲泼尼龙琥珀酸钠(甲强龙)冲击疗法治疗小儿肾病综合征( NS)的疗效及对肾功能指标、ADAM10、BMP-7水平的影响.方法 选取2015年1月至2018年1月在本院收治的NS患儿66例,根据治疗方法不同分为对照组和观察组.对照组患儿采用泼尼松治疗,观察组患儿采用甲泼尼龙琥珀酸钠(甲强龙)冲击治疗,分析两组患儿治疗后的临床效果.结果 观察组患儿总有效率为96. 97% ,对照组为81. 82% ,观察组患儿总有效率高于对照组,差异有统计学意义(P<0. 05).治疗前,两组患儿肾功能指标、ADAM10、BMP-7水平比较,差异无统计学意义(P>0. 05).治疗后,观察组患儿Scr、BUN、UA、ADAM10水平低于对照组,BMP-7水平高于对照组,差异有统计学意义(P<0. 05).治疗前,两组患儿血清和尿液中IL-18、TGF-β1水平比较,差异无统计学意义( P>0. 05).治疗后,观察组患儿血清和尿液中IL-18水平高于对照组,TGFβ1水平低于对照组,差异有统计学意义( P<0. 05).结论 甲泼尼龙琥珀酸钠(甲强龙)冲击疗法治疗NS 疗效显著,可改善患儿肾功能,降低ADAM10、TGF-β1 水平,提高BMP-7、IL-18水平,利于机体康复.
Objective To analyze the effect of methy 1 prednisolone shock therapy on renal function in children with nephrotic syndrome(NS).Methods From January 2015 to January 2018,66 cases of NS were divided into control group and observation group.Patients in the control group were treated with prednisone and those in the observation group were treated with methylprednisolone.Results The total effective rate of the observation group was 96.97%and that of the control group was 81.82%.The total effective rate of the observation group was higher than that of the control group(P<0.05).There was no significant difference in the level of BMP-7 between the two groups before treatment(P>0.05).After treatment,the level of ADAM10 in the observation gioup was lower than that in the control group(P<0.05),and the level of BMP-7 was higher than that in the control group(P<0.05).Before treatment,the levels of IL-18 TGF-β1 in serum and urine were measured in both groups.The difference was not statistically significant(P〉0.05).After treatment,the levels of IL-18 in serum and urine in the observation group were higher than those in the control group,and the level of TGF-01 was lower than that in the control group(P<0.05).Conclusions The effect of methylprednisolone pulse therapy on NS is significant.It can improve the renal function,decrease the level of TGF-β1 in serum and urine,increase the level of IL-18,and benefit the recovery of the body.
