摘要
目的评估胶囊内镜在小肠疾病中的诊断价值及其安全性。方法收集2014年1月至2018年12月在昆明医科大学第一附属医院消化内科行胶囊内镜检查的426例患者的疑诊小肠疾病的临床资料,回顾性分析胶囊内镜体内运行时间、通过幽门和小肠时间、胶囊内镜检查成功率、全小肠检查完成率和病变检出率。结果426例患者以住院患者为主(65.96%),其中1例(0.23%)因肠道准备差未能完成检查,12例(2.82%)滞留于胃腔,经胃镜引导至小肠。其中1例(0.23%)出现胶囊滞留,转至外科行手术治疗。全小肠检查成功率为99.53%,小肠疾病检出率为60.56%。检出的小肠病变以非特异性炎症为主,占35.44%。其对腹痛、小肠出血的检出率明显高于其他患者(P<0.05)。结论胶囊内镜是一种安全、有效的小肠疾病检查手段,尤其是对腹痛、小肠出血有较高的诊断价值。
Objective To evaluate the diagnostic value and safety of capsule endoscopy in small intestinal diseases.Me thods Clinical data of 426 patients with suspected small intestinal diseases in the Department of Gastroenterology of the First Affiliated Hospital of Kunming Medical University from January 2014 to December 2018 which had undergone capsule endoscopy were retrospectively collected and analyzed.Re s ults 426 patients were mainly hospitalized(65.96%).One case(0.23%)failed to complete the examination due to poor intestinal preparation,and 12 cases(2.82%)remained in the gastric cavity and were guided to the small intestine by gastroscopy.One case(0.23%)had capsule retention and was transferred to surgery.The success rate of total small intestinal examination was 99.53%,and the detection rate of small intestinal diseases was 60.56%.Non-specific inflammation accounted for 35.44%of the detected small intestinal lesions.The detection rates of abdominal pain and small intestinal bleeding were significantly higher than those of other patients(P<0.05).Conclus ion Capsule endoscopy is a safe and effective method for the examination of small intestinal diseases,especially for the diagnosis of abdominal pain and small intestinal bleeding.
