摘要
目的:观察温肾培元降毒汤结合中药灌肠对慢性肾功能衰竭患者肾动脉血流动力学和炎症状态的影响。方法:选择2017年1月~2018年7月我院肾内科收治的慢性肾衰竭患者86例,根据Excel随机表格法分为对照组和观察组,各43例;对照组采用常规西医治疗;观察组患者在此基础上加用温肾培元降毒汤结合中药灌肠对慢性肾功能衰竭患者进行治疗。两组患者共治疗4周。比较两组患者治疗疗效、比较两组患者治疗前后肾动脉血流动力学指标、比较两组患者治疗前后肾功能指标、比较两组患者治疗前后炎性因子水平。结果:研究结果显示,对照组总有效率为79.07%,观察组总有效率为95.35%,观察组总有效率明显高于对照组(P<0.05)。治疗后观察组舒张末期血流速度比较高于对照组,收缩末期血流速度、肾动脉阻力比较低于对照组(P<0.05)。治疗后观察组患者肾功能指标改善显著优于对照组(P<0.05)。治疗后观察组血清CRP、IL-6、IL-8水平对比低于对照组(P<0.05)。结论:综上所述总结得出,采用温肾培元降毒汤结合中药灌肠能提高慢性肾功能衰竭患者的治疗疗效,通过改善患者肾动脉血流动力学及炎症反应,从而起到对患者肾功能的改善,值得临床推广应用。
Objective:To study the effect of Wenshen Peiyuan Jiangdu Decoction combined with Enema of traditional Chinese medicine on renal artery hemodynamics and inflammation for patients with chronic renal failure. Methods:86 patients with chronic renal failure admitted to our hospital from January 2017 to July 2018 were divided into controlled group and observation group according to Excel random table method, 43 cases in each group. The controlled group was treated with conventional western medicine. The observation group patients were treated with Wenshen Peiyuan Jiangdu Decoction combined with enema of traditional Chinese medicine on this basis. Patients were treated. The two groups of patients were treated for 4 weeks. The therapeutic effect, renal artery hemodynamics, renal function and inflammatory factors were compared between the two groups before and after treatment. Results:The results showed that the total effective rate of the controlled group was 79.07%, and that of the observation group was 95.35%. The total effective rate of the observation group was significantly higher than that of the controlled group(P<0.05). After treatment, the end-diastolic blood flow velocity in the observation group was higher than that in the controlled group, while the end-systolic blood flow velocity and renal artery resistance in the observation group were lower than those in the controlled group(P<0.05). After treatment, the improvement of renal function in the observation group was significantly better than that in the controlled group(P<0.05). After treatment, the levels of CRP, IL-6 and IL-8 in the observation group were lower than those in the controlled group(P<0.05). Conclusion:In conclusion, Wenshen Peiyuan Jiangdu Decoction combined with traditional Chinese medicine enema can improve the therapeutic effect of patients with chronic renal failure, improve the renal artery hemodynamics and inflammatory reaction, thus improving the renal function of patients, which is worthy of clinical application.
