Infection characteristics of multiple drug-resistance bacteria in grass-roots hospitals of traditional Chinese medicine

摘要 目的探讨基层中医医院多重耐药菌(MDRO)分布特点,为医院感染控制提供参考依据。方法对2017年1月~2019年3月住院患者4060份标本进行需氧和真菌培养,并对培养出的阳性菌进行MDRO分析,观察MDRO的检出率,不同科室MDRO的分布、不同标本类型MDRO的分布。结果在送检的4060份标本中,检出阳性菌1325株,MDRO 162株,感染发生率为12.23%,MDRO感染中革兰阴性菌79.01%,革兰阳性菌20.99%。MDRO感染的主要细菌菌种为鲍曼不动杆菌40.74%,大肠埃希菌17.28%,屎肠球菌10.49%,肺炎克雷伯菌9.26%,铜绿假单胞菌9.26%。不同科室MDRO感染重症监护室(ICU)72.83%,内科系统15.44%,外科系统11.73%。不同标本MDRO感染痰液70.98%,尿液22.22%,伤口分泌物4.32%,血液2.48%。结论MDRO以鲍曼不动杆菌感染率最高,ICU检出率最高,痰液标本检出率最高。临床医师应重视病原学检查及药敏监测,加强对高危科室及易感人群的监控,减少MDRO的发生。 Objective To explore the distribution characteristics of multiple drug-resistance bacteria(MDRO)in grass-roots hospitals of traditional Chinese medicine hosital,and to provide the reference for the controlling of nosocomial infection.Methods The aerobic culture and fungal culture of 4060 specimens of inpatients were performed from January 2017 to March 2019.The MDRO analysis of the positive bacteria was performed.The detection rate of MDRO,the MDRO distributions in different departments and the MDRO distributions in different types of specimens were observed.Results Among 4060 samples,there were 1325 strains of positive bacteria and 162 strains of MDRO were detected,the incidence of infection was 12.23%.The MDRO infection rate of gramnegtive bacteria was 79.01%and that of gram-positive bacteria was 20.99%.The main bacterial strains of MDRO infection were Acinetobacter baumannii(40.74%),Escherichia coli(17.28%),Enterococcus faecium(10.49%),Klebsiella pneumoniae(9.26%)and Pseudomonas aeruginosa(9.26%).The MDRO infection distribution rates was 72.83%in intensive care unit(ICU),15.44%in internal medicine system and 11.73%in surgical system.MDRO infection rate was 70.98%in sputum,22.22%in urine,4.32%in wound secretion and 2.48%in blood.Conclusion For MDRO infection,acinetobacter baumannii infection rate was the highest,the detection rate was the highes in ICU and sputum samples.Clinicians should pay attention to pathogenic examination and drug sensitivity monitoring,strengthen the monitoring of high-risk departments and susceptible populations,and reduce the incidence of MDRO.
