摘要
目的:探究Nd:YAG激光治疗仪在不同病变类型后发性白内障中的应用效果及对患者血小板衍生生长因子A(PDGF-A)表达的影响。方法:回顾性分析医院收治的146例(146眼)后发性白内障患者的临床资料,其中Elschnig珍珠样小体型82例(82眼),纤维化型64例(64眼)。Elschnig珍珠样小体型患者采用“十”字形切开法,纤维化型患者采用开罐式切开法或“十”字形切开法,并根据不同病变类型选择激光参数治疗。于术后1周至6个月,复查视力、眼压及眼部情况。比较不同病变类型患者术后视力变化、并发症及PDGF-A阳性表达率。结果:纤维化型患者激光术后视力提高显著、有效、不变和下降分别为44例、18例、1例和1例,显著优于Elschnig珍珠样小体型患者的34例、42例、5例和1例,治疗效果差异有统计学意义(x^2=9.560,P<0.05);纤维化型患者术后发生眼压升高及人工晶体损伤的概率均低于Elschnig珍珠样小体型,差异有统计学意义(x^2=5.348,x^2=4.692;P<0.05);术后两种类型病变的患者PDGF-A阳性表达率均比术前显著降低,且纤维化型患者显著低于Elschnig珍珠样小体型,差异有统计学意义(t=4.256,P<0.05)。结论:Nd:YAG激光治疗可作为后发性白内障患者的主要治疗方法。
Objective:To investigate the application effect of Nd:YAG laser therapeutic instrument in the treatment of after-cataract with different types of lesions and its impact on the expression of platelet-derived growth factor-A(PDGF-A).Methods:The clinical data of 146 patients(146 eyes)with after-cataract who received treatment in hospital were retrospectively analyzed.Among them,82 cases(82 eyes)were Elschnig pearl-like small size type and 64 cases(64 eyes)were fibrosis type.Patients with Elschnig pearl-like small size type were treated by cross-shaped incision method while patients with fibrosis type were treated by can-opening incision or cross-shaped incision method.And different laser parameter treatments were selected according to different types of lesions.The eyesight,intraocular pressure and eye condition were reexamined from 1 week to 6 months post operation.The changes of postoperative eyesight,complications and the positive expression rate of PDGF-A among patients with different types of lesions were compared.Results:The improvement of eyesight in patients with fibrosis type(obvious result in 44 cases,effective result in 18 cases,unchanged result in 1 case,decreased result in 1 case)was significantly better than that in patients with Elschnig pearl-like small size type(obvious result in 34 cases,effective result in 42 cases,unchanged result in 5 cases,decreased result in 1 case)after laser surgery(x^2=9.560,P<0.05).The probabilities of intraocular pressure increasing and artificial lens damage post operation in patients with fibrosis type were significantly lower than those in patients with Elschnig pearl-like small size type(x^2=5.348,x^2=4.692,P<0.05),respectively.Compared with the positive expression rates of PDGF-A of patients with different types of lesions pre operation,those of them post operation were decreased significantly.And the rate in patients with fibrosis type was significantly lower than that in patients with Elschnig pearl-like small size type(t=4.256,P<0.05).Conclusion:Nd:YAG laser therapy can be used as the main therapeutic method for patients with after-cataract.
