摘要
目的 探讨同种输血和输血量与心脏手术术后感染的相关性。方法 选择 12岁以上心脏病人 2 6 6例 ,在静吸复合麻醉、低温心肺转流下行心脏外科手术。根据是否输用同种血和输血量分成三组 :A组 (n =71)为对照组 ,无同种输血 ;B组 (n =5 1)同种输血量≤ 4 0 0ml;C组 (n =14 4 )同种输血量≥ 80 0ml。对三组病例术后感染情况进行分析比较。结果 A组术后感染 2例 (2 81% ) ,B组感染 12例 (2 3 6 % ) ,C组感染 37例 (2 5 6 % ) ,B、C组感染率显著高于A组 (P <0 0 1)。结论 同种输血是心脏手术术后感染的危险因素之一 ,但是否是感染独立的相关因素尚需进一步研究。
Objective To examine the relation of infection with allogeneic blood transfusion and transfusion volume after cardiovascular surgery.Method 266 patients,12 years of age or older,were scheduled for elective cardiovascular surgeries under venous inhalation complex anesthesia and hypothermia cardiopulmonary bypass.Based on whether receiving allogeneic blood transfusion or not and transfusion volume,they were divided into three groups.Group A( n= 71):no allogeneic blood transfusion,as a controlled group;Group B ( n= 51):transfusion volume ≤400ml;Group C ( n= 144):transfusion volume≥800ml.The postoperative infections among three groups were compared and analysed.Result The study showed that 2 cases(2 81%),12 cases (23 6%) and 37 casese (25 6%)had postoperative infection in group A,B and C respectively.The infection rate in group B,C was significantly higher than that in group A( P< 0 01).Conclusion Allogeneic blood transfusion and transfusion volume may be one of the risk factors of postoperative infection in patients undergoing cardiovascular surgeries,but if it is an isolated relative factor of infection needs further investigations. [
