摘要
目的探讨超脉冲CO2点阵激光联合A型肉毒毒素注射治疗增生性瘢痕的临床疗效及安全性。方法自2018年6-12月,随机将30例增生性瘢痕患者分为3组,每组各10例;分别行皮损内A型肉毒毒素注射治疗(A组)、超脉冲CO2点阵激光治疗(B组)、皮损内A型肉毒毒素注射联合超脉冲CO2点阵激光治疗(C组)。以温哥华瘢痕量表(VSS)、视觉模拟评分法(VAS)进行评价分析,并采用两因素重复测量方差分析对比。结果 3组患者均完成了治疗疗程。随着治疗次数的增加,C组的VSS下降较快,与A组、B组比较,其差异有统计学意义(P<0.05)。C组与A组对瘢痕痛痒觉的改善情况相当,均好于B组(P<0.05)。C组与A组不良反应的发生率比较,其差异无统计学意义(P>0.05);但与B组比较,其差异有统计学意义(P<0.05)。结论采用超脉冲CO2点阵激光联合A型肉毒毒素皮损内注射治疗增生性瘢痕疗效更好,安全性较高,值得临床推广。
Objective To explore the therapeutic regimen of botulinum toxin type A injection combined with laser for hypertrophic scar skin lesions. Methods From June to December 2018, 30 patients with hypertrophic scars were randomly divided into3 groups, with 10 patients in each group. Botulinum toxin type A was injected into skin lesions in group A, ultra-pulsed CO2 fractional laser was performed in group B, and botulinum toxin type A injection combined with ultra-pulsed CO2 fractional laser was given in group C. The Vancouver scar scale(VSS) and visual analogue scale(VAS) were used for evaluation and analysis, and two-factor repeated measurement variance analysis was used for comparison. Results All patients completed the trial. With the increase of treatment times, patients in group C showed the fastest decrease on the VSS, which was significantly different from patients in group A and B(P<0.05). The improvement of pain and itching of scars in group C was similar to that in group A, which was, in turn, better than that in group B(P<0.05). There was no significant difference in the incidence rate of adverse reactions between groups C and A(P>0.05), but a significant difference was observed between groups C and B(P<0.05). Conclusion Ultra-pulsed CO2 fractional laser combined with botulinum toxin type A injection for hypertrophic scars has a better curative effect and greater safety, making it a valuable clinical option.
