摘要
目的:观察养心益气汤治疗2型糖尿病合并冠心病的疗效并分析其可能机制。方法:选取2016年3月~2018年3月我院收治的285例2型糖尿病合并冠心病患者作为研究对象。按照治疗方法将患者分为观察组(n=144)及对照组(n=141)。观察两组患者治疗后临床疗效。观察并记录治疗前、治疗1周、4周、8周两组患者中医症状评分。比较两组患者治疗前后血糖指标、氧化应激指标及炎症因子水平。结果:观察组患者治疗后总有效率为83.3%,明显高于对照组(63.8%,P<0.05)。两组患者治疗后中医症状评分有随时间变化下降的趋势,其中观察组下降程度高于对照组(P<0.05)。治疗后,两组患者HbA1c、FPG及2h PG水平均低于治疗前(P<0.05),其中观察组下降程度高于对照组(P<0.05)。治疗后,两组患者血清CAT、Gcl-C、SOD、脂联素水平均高于治疗前,MDA、hs-CRP、IL-6、TNF-α水平明显低于治疗前(P<0.05),其中观察组CAT、Gcl-C、SOD、脂联素下降程度高于对照组,MDA、hs-CRP、IL-6、TNF-α升高幅度高于对照组(P<0.05)。两组患者均未发生严重不良反应。结论:养心益气汤治疗2型糖尿病合并冠心病的疗效较好,能够明显降低患者血糖水平,改善患者临床症状,其机制可能与调节氧化应激因子及炎症因子水平相关。
Objective:To explore the curative effect of Yangxin Yiqi Decoction on type 2 diabetes mellitus complicated with coronary heart disease and analyze its possible mechanism.Methods:From March 2016 to March 2018,285 patients with type 2 diabetes mellitus and coronary heart disease were selected as the study subjects.Patients were divided into observation group(n=144)and controlled group(n=141)according to the treatment method.To observe the clinical effect of the two groups of patients after treatment.The scores of TCM symptoms were observed and recorded before treatment,1 week,4 weeks and 8 weeks after treatment.The levels of blood sugar,oxidative stress and inflammatory factors were compared between the two groups before and after treatment.Results:The total effective rate of the observation group was 83.3%,which was significantly higher than that of the controlled group(63.8%,P<0.05).After treatment,the scores of TCM symptoms in the two groups decreased with time,and the decrease degree in the observation group was higher than that in the controlled group(P<0.05).After treatment,the levels of HbA1 c,FPG and 2 HPG in the two groups were lower than those before treatment(P<0.05),and the decrease in the observation group was higher than that in the controlled group(P<0.05).After treatment,the levels of serum CAT,Gcl-C,SOD and adiponectin in the two groups were higher than those before treatment,and the level of MDA,hs-CRP,IL-6 and TNF-αwas significantly lower than that before treatment(P<0.05).The decrease of CAT,Gcl-C SOD and adiponectin in the observation group was higher than that in the controlled group,and the increase of MDA,hs-CRP,IL-6 and TNF-αwas higher than that in the controlled group(P<0.05).There were no serious adverse reactions in both groups.Conclusion:Yangxin Yiqi Decoction is effective in treating type 2 diabetes mellitus with coronary heart disease,it can significantly reduce blood sugar level and improve clinical symptoms of patients.Its mechanism may be related to the regulation of oxidative stress factors and inflammatory factors.
