摘要
肾纤维化是各种原发性和继发性慢性肾脏病(CKD)进展至终末期肾衰竭的共同途径。近年来,关于肾纤维化发病机制的研究取得了重大进展,但其诊断金标准仍为肾穿刺活检术。肾穿刺不仅对禁忌证患者不适用,还具有创伤性,且肾穿刺对早期肾纤维化诊断并不敏感。而肾纤维化生物标志物能弥补肾穿刺的不足。目前诊断肾纤维化的生物标志物种类繁多,在精准医学理念的指导下,从细胞和分子水平寻找肾纤维化诊断的特异性生物标志物,有助于早期干预肾纤维化的形成。未来,可联合应用多种生物标志物,以早期诊断肾纤维化。
Renal fibrosis is a common pathway for the progression of various primary and secondary chronic kidney diseases(CKD)to end-stage renal failure.In recent years,significant progress has been made in the study of the pathogenesis of renal fibrosis,but the diagnostic gold standard is still renal biopsy.Renal puncture is not only inapplicable to patients with contraindications,but also traumatic,and renal puncture is not sensitive for the diagnosis of early renal fibrosis.The renal fibrosis biomarkers can make up for the shortcomings of renal puncture.At present,there are many kinds of biomarkers for diagnosing renal fibrosis.Under the guidance of precise medical concept,the specific biomarkers for the diagnosis of renal fibrosis at the cellular and molecular levels are helpful for early intervention during the formation of renal fibrosis.In the future,multiple biomarkers can be combined to diagnose early renal fibrosis.
